Dear Mr. Thomas,
We are requesting a Nationwide 14 pernut for replacement of Bridge No. 79 with a triple barrel
box culvert on SR 1231 (Happy Hill Road) over Indian Grave Creek (Class C) in Davidson
County. I have attached a pre-construction notification application, construction plans, erosion
control plans, a stormwater management plan, and No Survey Required forms from NCDOT
Cultural Resources for Historic Architecture and Archeology. The proj ect will permanently
impact 1021inear feet and temporarily impact 18 linear feet of Indian Grave Creek. Stream flow
will be isolated into one barrel, with 2' sills and floodplain benches constructed at the inlet and
outlet of the overflow barrels in order to prevent stream over widening and facilitate aquatic
passage. The low flow barrel will be buried 1' below the stream bed to ensure aquatic passage.
The overflow barrels will be backfilled with class 1 rip rap to the sill elevation, and the low flow
barrel will be backfilled with class 1 rip rap to the stream bed elevation to expedite the
establishment of a natural stream bed. The culvert site will be dewatered during constnxction in
accordance with NCDOT's Best Management Practices.
By copy of this letter, we also request 401 review and approval from the NC Division of Water
Resources with one copy of the application package provided to Amy Chapman, Transportation
Pernutting Unit Supervisor, and Dave Wanucha, DOT Project Coordinator. Please charge the
application fee of $240 to WBS project no. 17BP.9.R.17.
By copy of this letter, we also request Marla Chambers, NC Wildlife Resources Commission, to
comment directly to the Corps concerning the permit request.

SUMMARy pF CULTiJRAL RESOURCES REVIEW
Brief description of review activities, results of review, and conclusions: HPO quad maps,
historic designations roster, and indexes reviewed on 8 April 2010 and yielded no NR, SL, LD,
DE, or SS properties in the Area of Potential Effects (pPE). The county architectural survey
(1981-2) and related publication recorded no properties in the APE (aaui satcerTouart, aui�diny
the Backrountry -An A�chitectura/ History ofDavidson Caunty, North Cara/ina ([Lexington, NC7: Davidson County
Historiwi association, isa�)). Three surveyed sites stand nearby on non-adjacent parcels: the
mid-nineteenth-century George Koontr House (DV 336) about 1250 feet northeast of the
bridge, the mid-nineteenth-century Riley Leonard House (DV 335) about 1100 feet east of
the bridge, and the circa-1870 Lindsay Leonard House (DV 275} about 725 feet west of the
bridge. Davidson County current GIS mapping, aerial photography, and tax information
indicate wooded and developed parcels adjacent to the bridge, including buildings dating to
the 1970s-1990s. Constructed in 1950, Bridge No. 79 is a single-span, 31-foot, steel
stringer/multi-beam bridge and is not eligible for the National Register according to the
NCDOT Historic Bridge Survey, as it is not historicatly, architecturally, or technologiwlly
significant. Google Maps "Street View" confirmed absence of historic structures/landscapes
in APE (viewed 8 April 2010). No architectural survey required for project as currently
def ned.
BriefExpZanation ofwhy the available informcttion provides a reliable fiasis for reasonably predicting
that there are no unidentified historic properties in the APE: ApE centered on existing bridge and
extends 40 feet from centerline to the S and N and 200 feet from either end of the bridge to
the W and E. While not recent, the county survey is well-executed and rewrds no properties
in the APE. County GIS/tax materials and other images support absence of significant
architectural resources. No National Register-listed or —eligible properties or other
properties of concern are located within the APE.

The purpose of this letter is to notify you that the Ecosystem Enhancement Program (EEP) will provide the
compensatory stream mitigation for the subject project. Based on the in£ormation received from you on Febmary 4,
2014, the impacts are located in CU 03040103 of the Yadkin River basin in the Cenu-a] Piedmont (CP) Eco-Region,
and are as follows:

This impact and associated mitigation need were under projected by the NCDOT in the 2013 impact data.
EEP will commit to implement sufficient compensatory stream mitigation credits to offse[ the impacts associated
with this project as detemuned by the regulatory agencies using the delivery timeline listed in Section F.3.c.iii of the
N.C. Department of Enduonment and Natural Resources' F.cosystem Enhancement Program In-Lieu Fee Instrument
dated July 28, 2010. If the above referenced impact amounts are revised, then this mitigation accepta�ce letter will
no longer be valid and a new mirigation acceptance letter wIll be required from EEP.
