Per your narrative, this project is relocating an existing sewer line, and you indicated as such in Part II, Item 2 on the FTA 06-21 form. However, no existing permit number was included in the application. If this line was previously permitted, I will need the permit number (e.g. WQ00XXXXX) that was assigned to the line upon initial approval by the DWR, as well as the additional items named under Part G of the Instructions for Form: FTA 06-21 (the three page checklist). This will allow me to alter that existing permit and document all changes to the system. If, alternatively, no permit number exists for this line (as is the case with some lines installed more than a few decades ago, prior to our division issuing permits for sewers), I will need you to resubmit the application as a "Proposed (New Permit)", as indicated in Part II, Item 2 of the FTA form. This will allow me to issue this permit with a new number by which it can be identified. In addition to these updates, regardless of the permit status, I will need the "Approximate Coordinates" of the project, which should be included in Part II, Item 4. Lastly, I will need you to check the box in Part X, Item 1, to complete the Certifications page. As soon as you can get this to me I will be able to begin drafting an updated permit.
