The report period is from July 1, 2000 to June 30, 2001 on a twelve (12) months of performance. Discharge Permit No. NC0024571. � Q N 00 [ N Was waters and-isbase Wastewater Treatment Plant: The Wastewater facility is located at 700 LaFayette Street, Lumberton, North Carolina. The operator in responsible charge is Jon W. Locklear. Persons needing any additional information concerning this report can call the Wastewater Treatment Plant at (910) 671-3859. City of Lumberton Collection System: The Collection System is located in and around the City of Lumberton. Responsible persons in charge are H. Dixon Ivey and Howard Revels. H. Wastewater Treatment Plant: All domestic and industrial wastewater generated by customers is treated at the facility with a daily discharge effluent to the Lumber River. The discharge effluent is tested and monitored per permit regulations NC0024571. The facility reports monthly to the North Carolina Department of Environment and Natural Resources. City of Lumberton Collection System: The City of Lumberton Collection System has approximately 200 miles of piping and 38 lift stations. Lift Stations are monitored daily and operate under Permit No. NC0024571. Wastewater Treatment plant Performance: The Wastewater Treatment Plant did perform satisfactorily for the report period. The facility processes were operated and maintained with various structures receiving sandblasting and painting. The facility had only three (3) months with permit violation and three (3) months with monitoring and reporting violations. The violations did not have any known impact on the receiving stream (Lumber River). The following is a summary of the months with violations and description of any corrective measures taken. August 2000: Permit violation for not meeting dissolved oxygen permit limits on August 11, 2000. Cause: Unknown. Corrective Action: Laboratory staff will recalibrate and recheck to ensure the dissolved oxygen meter is reading correctly. Monitoring and reporting violation for no influent sample on August 13, 2000. Cause: Influent sampler malfunctioned and failure to check sampler, human error. Corrective Action: Replaced influent sampler and staff required using a checklist for monitoring equipment. November 2000: Monitoring and reporting violation for no test results from ammonia effluent samples on November 29, 2000. Cause: Insufficient sample volume from effluent sampler. Corrective Action: None November 2000: Permit violation for exceeding fecal coliform permit limits on November 6, 2000 and November 7, 2000. Cause: Facility was experimenting with Hydrogen Peroxide as a disinfectant. Corrective Action: Stopped experimenting with Hydrogen Peroxide and continued use of Chlorine as method of disinfecting. February 2001: Permit violation exceeding weekly fecal coliform permit limits on February 12, 2001 and February 16, 2001. Cause: Unknown. Corrective Action: None April 2001: Monitoring and reporting violation for no effluent composite sample on April 15, 2001. Cause: Sampler tubing ruptured. Corrective Action: Staff will start sampling equipment in the AM and will recheck in the PM to verify equipment operating properly. City of Lumberton Collection System Performance: The City of Lumberton Collection performed satisfactorily for the report period. The collection system was operated and maintained on a daily basis. Repairs to the system were done as required to maintain the system. Lift stations were monitored on a daily basis and maintenance was performed to upgrade and maintain pumping performance. We have installed a new SCADA system to help us monitor the operation of the lift stations and collection system. We currently have thirteen (13) lift stations with SCADA on-line and we plan to have five (5) more lift stations on-line this budget year. We have also installed backup generators on six (6) lift stations and we plan to install eleven (11) more generators and have one (1) portable generator this budget year. July 2000: Sanitary sewer overflows occurred on 07-13-00 at Swann Drive (4050 gals.); Edens Avenue (2700 gals.); Campbell Street (2700 gals.); Lincoln Street (1350 gals.); Stephens Park (16200 gals.) / (12000 gals. contained, not reaching surface waters). Cause: Heavy Rain. Corrective Action Taken: Areas were cleaned and disinfected. Overflows occurred on 07-25-00 at Swann Drive (3200 gals.); Edens Avenue (2100 gals.); Campbell Street (2100 gals.); Lincoln Street (1000 gals.); Stephens Park (12600 gals.) / (10000 gals. contained, not reaching surface waters). Cause: Heavy Rain. Corrective Action Taken: Areas were cleaned and disinfected. August 2000: Sanitary sewer overflows at Hampstead Road (900 gals.). Cause: Sewer line caved in. Corrective Action Taken: Hauled dirt out of area, disinfected with clorox and lime, and repaired sewer line. September 2000 Sanitary sewer overflows occurred on 09-18-00 at Swann Drive (150 gals.); Stephens Park (900 gals.). Cause: Heavy Rain. Corrective Action Taken: Areas were cleaned and disinfected. Overflow at Kahn Drive (1000 gals.) / (300 gals. contained, reaching surface waters) occurred 09-28-00. Cause: Broken force main. Corrective Action Taken: Pumps were cut oft area vacuumed, soil removed, new soil placed and repaired force main. October 2000 Sanitary sewer overflows at Swann Drive (250 gals.) / (250 gals. contained, not reaching surface waters) occurred 10-02-00. Cause: Blocked sewer line. Corrective Action Taken: Line Cleaned. Contained in ditch, vacuumed and disinfected ditch. 2 November 2000 Sanitary sewer overflows at Britts Park Branch Street (500 gals.) / (500 gals. contained, not reaching surface waters) occurred 11-04-00. Cause: Power failure at lift station. Corrective Action Taken: Area was cleaned, disinfected and power restored. January 2001 Sanitary sewer overflows occurred on 01-01-01 at I-95 and Hwy. #301 (150 gals.) / (50 gals. contained, not reaching surface waters). Cause: Leak at force main. Corrective Action Taken: Repaired force main, vacuumed, cleaned and disinfected. Overflows at occurred on 01-29-01 at Jasper Textiles on Mill Street (200 gals.) / (200 gals. contained, not reaching surface waters). Cause: Contractor broke sewer line. Corrective Action Taken: Area was cleaned, disinfected and line repaired. February 2001 Sanitary sewer overflows occurred on 02-14-01 off of Alamac Road (20000 gals.). Cause: Broken pipe on air release valve at lift station due to vandalism. Corrective Action Taken: Area cleaned up, disinfected and pipe repaired. March 2001 Sanitary sewer overflows occurred on 03-21-01 at Kasyer Roth Lift Station (600 gals.) Cause: Lift station power loss in control panel. Corrective Action Taken: Spill contained in waste lagoon. Overflow occurred on 03-28-01 at Contempora Drive Lift Station (200 gals.) / (200 gals. contained, not reaching surface waters). Cause: Galvanized nipple broke on air release valve. Corrective Action Taken: Area cleaned up, disinfected and power restored. Overflows occurred on 03-28-01 at Nash Finch Lift Station (900 gals.) / (900 gals. contained, not reaching surface waters. Cause: Main disconnect opened; vandalism. Corrective Action Taken: Area was cleaned, disinfected, and power restored. Overflows occurred on 03-30-01 behind 2114 East 7th Street (25 gals.) / (25 gals. contained, not reaching surface waters). Cause: Blockage in line. Corrective Action Taken: Area was cleaned vacuumed, disinfected, and cleaned line. May 2001 Sanitary sewer overflows occurred on 05-01-01 at Fayetteville Road and Godwin Avenue (900 gals.) / (300 gals. contained, not reaching surface waters). Cause: Contractor hit force main. Corrective Action Taken: Dammed adjacent ditch to canal, vacuumed area, disinfected and force main repaired. Overflows occurred on 05-02-01 at Emory Circle and Townsend Street (300 gals.) / (300 gals. contained, not reaching surface waters. Cause: Manhole overflowed blocked sewer line. Corrective Action Taken: Vacuumed, cleaned line and disinfected. Overflows occurred on 05-21-01 at Iveys Pump Station on South Street (100 gals.). Cause: Air pump failure in lift station. Corrective Action Taken: Sprayed area with disinfectant and air pump replaced. June 2001 Sanitary sewer overflows occurred on 06-02-01 at Stephens Park (600 gals.) / (600 gals. contained, not reaching surface waters). Cause: Heavy Rain. Corrective Action Taken: Spill was contained in low areas around manhole, cleaned and disinfected. Overflows occurred on 06-03-01 at 5204 Simmons Drive (20 gals.) / (20 gals. contained, not reaching surface waters). Cause: Lift Station phase monitor failure. Corrective Action Taken: Area cleaned, disinfected and phase monitor repaired. Overflows occurred on 06-04-01 at Hwy. 301 and I-95 (500 gals.) / (500 gals. contained, not reaching surface waters). Cause: Contractor hit line. Corrective Action Taken: Spill was contained in a ditch and pumped back into manhole, area cleaned, vacuumed, disinfected and line repaired. Overflows occurred on 06-09-01 at Sterling Drive (300 gals.). Cause: Lift station pump failure. 3 Corrective Action Taken: Repaired pump and disinfected with clorox. Overflows on 06-09-01 at Noir Street (4000 gals.); Edens Avenue (1500 gals.); Campbell Street (1725 gals.); Swann Drive (1950 gals.); Stephens Park (6500 gals.) / (6500 gals. contained, not reaching surface waters); West 5th Street (400 gals.) / (400 gals. contained, not reaching surface waters). Cause: Heavy Rain. Corrective Action Taken: Area cleaned and disinfected. Overflows occurred on 06-16-01 at Noir Street (7000 gals.); Edens Avenue (3200 gals.); Campbell Street (2300 gals.); Swann Drive (4000 gals.); Lincoln Street (680 gals.); Stephens Park (54,000 gals.) / (27,000 gals. contained, not reaching surface waters); Sterling Drive (6700 gals.). Cause: Heavy Rain. Corrective Action Taken: Area cleaned and disinfected. Overflows occurred on 06-16-01 at Sterling Drive Lift Station (700 gals.). Cause: Lift station blown fuse. Corrective Action Taken: Area cleaned, disinfected and fuse replaced. Notification Statement The City of Lumberton has advertised for one week in The Robesonian, local newspaper, and in the monthly bills that the Annual Report for the Wastewater Treatment and City of Lumberton Collection System will be on file at the City Clerks Office, 501 West 5th Street; Lumberton, North Carolina 28358.
