General Information Facility/System Name: Responsible Entity: Person in Charge/Contact: Applicable Permit(s): Performance Annual Report July 1, 2001—.June 30, 2002 ''.14H 1 8 2003 d Ellerbe Wastewater Treatment Plant • Town of Ellerbe Wendell Robinson 910-652-6251 NPDES Permit NC0021784 Description of Treatment Process: Operationand maintenance of 180,000 gallon per day wastewater treatment facility consisting of three lagoons, chlorine contact chamber (for disinfection), and post aeration. The facility is located on Bennett Road (behind the County's compactor site) approximately 1 % miles northwest of Town. The discharge from this facility is to Tom's Branch, classified as "C" waters in the Yadkin/Pee Dee River Basin. II. Performance Text Summary of System Performance for the period July 1, 2001 through June 30, 2002 Permit Limit Violations for chronic toxicity and fecal coliforms No Monitoring or Reporting Violations No Sanitary Sewer Overflows List any violations of permit conditions or other environmental regulations. Monthly lists should include discussion of any environmental impacts and corrective measures taken to address violations. September — Weekly average fecal coliform limit of 400/100 ml exceeded once April — Chronic toxicity value of61 % violated June — Chronic toxicity value of 61 % violated The Town replaced a faulty regulator, tubing, and a tank valve on the chlorine feed system soon after the September violation. The Town began re -testing chronic toxicity in May, passed the first of two serial dilution with a NOEC of 61% then failed the second re -test with a NOEC of <15.25%. The Town spread straw acrossthe surface of the third lagoon in an effort to eliminate the algae in the lagoon, which has generally been the reason for chronic toxicity failures in the past. The Town has also implemented clean sampling techniques. The Town passed the chronic toxicity test on samples taken during July. We are not aware of any environmental impacts as a result of these violations. III. Notification State how this report has been made available to users or customers of the system and how those users have been notified of its availability. Public notice placed in local newspaper summarizing this report and informing the public that a copy of this report can be obtained at the Ellerbe Town Hall. . IV. Certification I certify under penalty of law that this report is complete and accurate to the best of my knowledge. I further certify that this report has been made available to the users or customers of the Town of. Ellerbe wastewater treatment facility and that those users have been notified of its availability. tiO set,set,c6 Qa 6L1- Public Works Director, Town of Ellerbe Date Public Notice The North Carolina Clean Water Act of 1999 requires that each owner or operator of any wastewater treatment plant or wastewater collection system to provide an annual performance report to its users and customers. The Town of Ellerbe owns and operatesour wastewater collection system and wastewater treatment plant located on Bennett Road (NCSR 1310). A copy of the Town's performance annual report for the period July 1, 2001 through June 30, 2002 can be obtained at the Ellerbe Town Hall, located at 108 West Page Street, Ellerbe, North Carolina. This report states that, during the annual report period, the Town of Ellerbe had permit limit violations for chronic toxicity during April and June of 2002 and fecal conforms during September 2001. The Town made repairs to chlorination equipment soon after the fecal coliform violation in September. The Town implemented the required monthly re -testing protocol for chronic toxicity (the failure in June was on a re -test).: The Town subsequently passed the chronic toxicity test on samples taken during July.
