This Notice of Deficiency is being sent to you for the following deficiencies discovered for the
subject facility:
Failure to submit monthly pump & haul reports as required in Permit Condition II.1 of Permit No.
WQ0042770. The October monthly report was submitted late on December 3, 2021.
Please be advised that any future violations may result in the issuance of a Notice of Violation and
are subject to a civil penalty assessment.
