This permit shall be effective from the date of issuance until rescinded and you shall be subject to the conditions and limitations as specified in the North Carolina Gravity Sewer Design Criteria adopted February 12, 1996 and the attached permit. In accordance with the provisions of Article 21 of Chapter 143, General Statutes of North Carolina as amended, and other applicable Laws, Rules, and Regulations, permission is hereby granted for the construction and operation of approximately 333 linear feet of 8 inch gravity sewer to serve 20.83 acres at Monroe Corporate Center, Phase II, and the discharge of 10,415 GPD of collected domestic wastewater into the City of Monroe's existing sewerage system, pursuant to the application received August 11, 1997, and in conformity with the North Carolina Gravity Sewer Design Criteria. The sewage and wastewater collected by this system shall be treated in the City of Monroe Wastewater Treatment Facility (NPDES Permit No. NC0024333) prior to being discharged into the receiving stream. If any parts, requirements, or limitations contained in this permit are unacceptable, you have the right to request an adjudicatory hearing upon written request within thirty (30) days following receipt of this permit. This request must be in the form of a written petition, conforming to Chapter 150B of North Carolina General Statutes, and filed with the Office of Administrative Hearings, P.O. Drawer 27447, Raleigh, NC 27611-7447. Unless such demands are made this permit shall be final and binding. The issuance of this permit does not preclude the Permittee from complying with any and all statutes, rules, regulations, or ordinances which may be imposed by other government agencies (local, state, and federal) which have jurisdiction. This permit shall become voidable unless the facilities are constructed in accordance with the conditions of the Division of Water Quality Gravity Sewer Design Criteria adopted February 12, 1996. 2. This permit is effective only with respect to the nature and volume of wastes described in the application and other supporting data. 3. The facilities must be properly maintained and operated at all times. 4. This permit is not transferable. In the event there is a desire for the facilities to change ownership, or there is a name change of the Permittee, a formal permit request must be submitted to the Division of Water Quality accompanied by an application fee, documentation from the parties involved, and other supporting materials as may be appropriate. The approval of this request will be considered on its merits and may or may not be approved. 5. Construction of the gravity sewers shall be scheduled so as not to interrupt service by the existing utilities nor result in an overflow or bypass discharge of wastewater to the surface waters of the State. 6. Upon completion of construction and prior to operation of this permitted facility, a certification and a copy of the record drawings (i.e., as constructed plans) must be received from a professional engineer certifying that the permitted facility has been installed in accordance with the Division of Water Quality Gravity Sewer Design Criteria adopted February 12, 1996, and other supporting materials. Mail the Certification and one (1) copy of the "Record Drawings" to the Water Quality Permits and Engineering Unit, P.O. Box 29535, Raleigh, NC 27626-0535. 7. A copy of the Record Drawings shall be maintained on file by the Permittee for a minimum of five years from the date of the completion of construction. 8. Failure to abide by the conditions and limitations contained in this general sewer permit or the North Carolina Gravity Sewer Design Criteria may subject the Permittee to an enforcement action by the Division of Water Quality, in accordance with North Carolina General Statute 143-215.6(a) to 143- 215.6(c). 9. In the event that the facilities fail to perform satisfactorily, including the creation of nuisance conditions, the Permittee shall take immediate corrective action, including those as may be required by this Division, such as the construction of additional or replacement wastewater collection facilities. 1 10. Noncompliance Notification: The Permittee shall report by telephone to the Mooresville Regional Office, telephone number (704) 663-1699, as soon as possible, but in no case more than 24 hours or on the next working day, following the occurrence or first knowledge of the occurrence of either of the following: a. Any process unit failure, due to known or unknown reasons, that renders the facility incapable of adequate wastewater transport, such as mechanical or electrical failures of pumps, line blockage or breakage, etc.; or b. Any failure of a sewer line resulting in a by-pass directly to receiving waters without treatment of all or any portion of the influent to such station or facility. Persons reporting such occurrences by telephone shall also file a written report in letter form within five (5) days following first knowledge of the occurrence. This report must outline the actions taken or proposed to be taken to ensure that the problem does not recur. Permit issued this the 19th day of August, 1997. For Low -Pressure Air Testing, the test pressure shall be a minimum of 3.5 psi. Testing time in minutes shall be according to the schedule shown in Table SP-1. The segment of pipe being tested will be considered to pass if the air pressure drop does not exceed 0.5 psi at the end of the testing time. For lengths of pipe greater than 450 feet, the following formula shall be used for determining testing time: T = 0.000198 D2 L Where: T = Minimum testing time, minutes D = Nominal inside pipe diameter, inches L = Length of pipe tested, feet B. For exfiltration water test, following maximum absorption of the pipe, the test shall be conducted for a minimum of 4 hours. The acceptable drop of the water level in the upstream manhole shall be calculated based upon the diameter of the manholes, the size of the pipe, and the length of the pipe tested to achieve the standard of not greater than 50 gallons/mile/in. dia./day. For four -foot diameter manholes, a pipe size of 8", and a pipe length in the test section of 640 feet, the drop in the liquid level in the measured manhole shall not exceed 1" in 4 hours. C. Infiltration and exfiltration shall not exceed 50 gallons/mile/inch dia./day. 1.03 Section 5K, Manhole Construction, of the Standard Specifications shall be amended as specified below: A. All manholes for 8" sewer main shall be four (4') feet inside diameter, All manholes constructed with an invert separation of 2.5 feet or greater shall include an outside drop. Rim elevations of all manholes shall be as shown on the plans. Manholes with rim elevations below the 100-year flood elevation shall have sealed watertight lids. These locations are shown on the plans. Outside vents for watertight lid manholes shall be provided where shown on the plans and shall conform to the plan detail with the opening of the vent at least one foot above the 100-year flood elevation. 1.04 General Separation Requirements All gravity sewer mains shall be constructed such as to provide the following minimum separations: a) Vertical separation from storm sewers 12 inches b) Vertical separation from water mains where constructed under the water main 18 inches c) Horizontal separation from water mains 10 feet d) Vertical separation from benched trenches 18 inches e) Horizontal distance from any private or public water supply source, including any WS-1 waters or Class I or Class II impounded reservoirs used as a source of drinking water 100 feet f) Horizontal distance from waters classified WS-II, WS-III, B, SA, ORW, HQW, or SB from normal high water 50 feet g) Any other stream, lake, or impoundment, from normal high water 10 feet Where the required minimum separations stated above cannot be maintained, the sewer main shall be installed with minimum Pressure Class 150 ductile iron pipe including pressure joints in conformance with AWWA Specifications C150, C151, and C111. Using ductile iron pipe and pressure joints as stated immediately above, minimum separations of 25 feet from a private well and 50 feet from the normal high water line of a public water supply source shall still be maintained. 1.05 Creek Crossings For gravity sewers constructed across creeks, the sewer pipe elevation shall be such that the top of the pipe is located at or below the bottom of the stream, or the bottom of the pipe is located above the 25-year flood elevation. Construction shall conform to the detail shown on the plans. Pipe for stream crossings shall be ductile iron pipe pressure class 150 or high strength steel pipe. Transitions to PVC pipe shall be located a minimum of 18 feet horizontally (at least one full joint) beyond the stream bank and the sewer main at the transition location shall have at least 3 feet of bury. Each joint of the exposed pipe for aerial stream crossings shall be anchored and strapped to a reinforced concrete pier detailed by and • sealed by a structural engineer, as shown on the plans, to withstand bouyant forces and the fateral forces imposed by the stream current during the 100-year storm event.
