Enclosed please find a copy of the Collection System Inspection Report from the inspection conducted on June 3, 2014 by
Hughie White of the Fayetteville Regional Office. Mr. Ernie Billingsley, ORC, assisted with this inspection and his
assistance was very much appreciated. The facility was found to be in Compliance with permit WQCS00126.

Inspection Summary:
All records and log books were very well organized and appeared complete and current. A copy of the collection system
permit, spill response plan, sewer use ordinance, map of the system and capital improvement plan were all available for
review. The Wooten Co. is currently in the process of developing a new capital improvement plan for the City.
High -priority lines are being inspected at least once per every six month period of time, as required. The annual 10% of
line cleaning was met for the previous year. Right-of-ways are currently accessible in the event of an emergency. All
pump stations that were visited during this inspection appeared to be operated and maintained properly with very good
housekeeping.

The City uses a foaming chemical root treatment called RootX. The City performs the actual treatment
in-house.
Comment: The Wooten Company is currently developing a new capital improvement
plan for the City.
