A Division of Energy, Mineral, and Land Resources file review has determined that
Stormwater Permit SW7980823 for a stormwater treatment system consisting of a wet
detention pond serving the Holiday Inn Express located at 840 US Hwy 64, Plymouth,
NC expires on December 13, 2021. This is a reminder that permit renewal applications
are due 180 days prior to expiration. We do not have a record of receiving a renewal
application.
Please submit a completed permit renewal application along with a $505.00 fee for
permit renewal within the required time. Permit application forms for renewal can be
found on our website at: https://deq.nc.gov/about/divisions/energy-mineral-and-land-
resources/stormwaterlstormwater-program/post-construction-2. North Carolina General
Statutes and the Coastal Stormwater rules require that this property be covered under a
stormwater permit. Failure to maintain a permit subjects the owner to assessment of

We are returning check number 19260 in the amount of $550 (enclosed). Please issue a
check in the amount of $505 to NCDENR for the stormwater renewal fee. Also, I was able to
locate a Wet Detention Basin Operation and Maintenance Agreement in the project folder for
the Holiday Inn Express — Plymouth. Therefore, this particular form doesn't need to be
completed. The only form that still needs to be completed for the renewal process is the
Designer's Certification (enclosed) and it does need to be completed by a registered
professional engineer in the State of North Carolina. The original consulting engineer for the
permit was Jarvis Associates (here in Washington) in case your company would like to contact
them to see if they ever completed a Designer's Certification for the Holiday Inn Express in
Plymouth. They (Jarvis Associates) may have this form on file. If they don't, either Jarvis
Associates or another engineer of your company's choice will need to complete this enclosed
form for the renewal process to begin.

In reference to the inspection of our wet detention pond I have scheduled the following to
be started on Friday January 4,2008. I have hired a local contractor to come in and
remove all cattails from the pond, he will be having a load of rock delivered to rebuild the
erosion at the sides of the ponds, all trash debri and any other sediments will be skimmed
and removed. He will be replacing the trash rack on the top of outlet riser. There will be
a maintenance upkeep record book kept from this day forward updated as needed. Thank
you for your assistance on this matter and I will keep you informed as the project is
completed.

The Washington Regional Office received the completed
Stormwater Anplication for the subject project on November 18,
1998. Staffreview of the plans and specifications has determined
that the project, as proposed, will comply with the Stormwater
Regulations set forth in Title 15A NCAC 2H.1000. We are forwarding
Permit No. SW7980823 dated December 28, 1998 to Plymouth
Hospitality, LLC.
This permit shall be effective from the date of issuance until
December 28, 2008 and shall be subject to the conditions and
limitations as specified therein. Please pay special attention to
the Operation and Maintenance requirements in this permit. Failure
to establish an adequate system for operation and maintenance of
the stormwater management system will result in future compliance
problems.
If any parts, requirements, or limitations contained in this
permit are unacceptable, you have the right to request an
adjudicatory hearing upon written request within thirty (30) days
following receipt of this permit. This request must be in the form
of a written petition, conforming to Chapter 150B of the North
Carolina General Statutes, and filed with the Office of
Administrative Hearings, P.O. Drawer 27447, Raleigh, NC 27611-7447.
Unless such demands are made this permit shall be final and
binding.

I. DESIGN STANDARDS
1. This permit is effective only with respect to the nature and
volume of stormwater described in the application and other
supporting data.

4. No homeowner/lot owner/developer shall be allowed to fill in,
alter, or pipe any vegetative practices (such as swales) shown
on the approved plans as part of the stormwater management
system without submitting a revision to the permit and
receiving approval from the Division.
5. The following items will require a modification to the permit:
a. Any revision to the approved plans, regardless of size
b. Project name change
C. Transfer of ownership
d. Redesign or addition to the approved amount of built -upon
area
e. Further subdivision of the project area.
In addition, the Director may determine that other revisions
to the project should require a modification to the permit.
6. The Director may notify the permittee when the permitted site
does not meet one or more of the minimum requirements of the
permit. within the time frame specified in the notice, the
permittee shall submit a written time schedule to the Director
for modifying the site to meet minimum requirements. The
permittee shall provide copies of revised plans and
certification in writing to the Director that the changes have
been made.
II. SCHEDULE OF COMPLIANCE
1. The permittee will comply with the following schedule for
construction and maintenance of the stormwater management
system.
a. The stormwater management system shall be constructed in
its entirety, vegetated and operational for its intended
use prior to the construction of any built -upon surfaces
except roads.
b. During construction, erosion shall be kept to a minimum
and any eroded areas of the system will be repaired
immediately.
2. The facilities must be properly maintained and operated at all
times. The approved Operation and Maintenance Plan must be
followed in its entirety and maintenance must occur at the
scheduled intervals.
3. The permittee shall at all times provide the operation and
maintenance necessary to assure the permitted stormwater
system functions at optimum efficiency including, but not
limited to:

a. Semi-annual scheduled inspections (every 6 months) v
b. Sediment removal
C. Mowing and revegetation of side slopes
d. Immediate repair of eroded areas
e. Maintenance of side slopes in accordance with approved
plans and specifications
.f. Debris removal and unclogging of outlet structure,
orifice device and catch basins and piping.
4. Records of maintenance activities must be kept and made
available upon request to authorized personnel of DWQ. The
records will indicate the date, activity, name of person
performing the work and what actions were taken.
5. This permit shall become voidable unless the facilities are
constructed in accordance with the conditions of this permit,
the approved plans and specifications, and other supporting
data.
6. Upon completion of construction and prior to operation of this
permitted facility, a certification must be received from an
appropriate designer for the system installed certifying that
the permitted facility has been installed in accordance with
this permit, the approved plans and specifications, and other
supporting documentation. Mail the Certification to the
Washington Regional Office, 943 Washington Square Mall,
Washington, North Carolina, 27889, attention Division of Water
Quality.
7. A copy of the approved plans and specifications shall be
maintained on file by the Permittee for a minimum of five
years from the date of the completion of construction.
III. GENERAL CONDITIONS
1. This permit is not transferable. In the event there is a
desire for the facilities to change ownership, or there is a
name change of the Permittee, a formal permit request must be
submitted to the Division of Water Quality accompanied by an
application fee, documentation from the parties involved, and
other supporting materials as may be appropriate. The
approval of this request will be considered on its merits and
may or may not be approved.
2. Failure to abide by the conditions and limitations contained
in this permit may subject the Permittee to enforcement action
by the Division of Water Quality, in, accordance with North
Carolina General Statute 143-215.6(a) to 143-215.6(c).
3. The issuance of this permit does not preclude the Permittee
from complying with any and all statutes, rules, regulations,
or ordinances which may be imposed by other government
agencies (local, state, and federal) which have jurisdiction.

4. In the event that the facilities vfail to perform
satisfactorily, including the creation of nuisance conditions,
the Permittee shall take immediate corrective action,
including those as may be required by this Division, such as
the construction of additional or replacement stormwater
management systems.
5. The permit may be modified, revoked and reissued or terminated
for cause. The filing of a request for a permit modification,
revocation and reissuance or termination does not stay any
permit condition.

WET DETENTION BASIN OPERATION AND MAINTENANCE AGREEMENT
Maintenance activities shall be performed as follows:
1. After every significant runoff producing rainfall event and at least monthly:
a. Inspect the wet detention basin system for sediment accumulation, erosion, trash accumulation,
vegetated cover, and general condition.
b. Check and clear the orifice of any obstructions such that drawdown of the temporary pool occurs within
2 to 5 days as designed.
2. Repair eroded areas immediately, re -seed as necessary to maintain good vegetative cover, mow vegetative
cover to maintain a maximum height of six inches, and remove trash as needed.
3. Inspect and repair the collection system (i.e. catch basins, piping, swales, riprap, etc.) quarterly to maintain
proper functioning.
4. Remove accumulated sediment from the wet detention basin system semi-annually or when depth is reduced to
75% of the original design depth (see diagram below). Removed sediment shall be disposed of in an
appropriate manner and shall be handled in a manner that will not adversely impact water quality (i.e.
stockpiling near a wet detention basin or stream, etc.).
The measuring device used to determine the sediment elevation shall be such that it will give an accurate
depth reading and not readily penetrate into accumulated sediments.

5. Remove cattails and other indigenous wetland plants when they cover 50% of the basin surface. These
plants shall be encouraged to grow along the vegetated shelf and forebay berm.
6. If the basin must be drained for an emergency or to perform maintenance, the flushing of sediment through
the emergency drain shall be minimized to the maximum extent practical.
7. All components of the wet detention basin system shall be maintained in good working order.
8. Remove all trees and heavy woody vegetation from the basins and dike walls.

You should be aware that failure to provide the Designer's Certification and the operation of a
stormwater treatment facility without a valid permit, are violations of NC General Statute 143-215.1 and
may result in appropriate enforcement action including the assessment of civil penalties of up to
$10,000 per day.

A. DESCRIPTION OF PROJECT
Location of Project
This site is located within the city limits of Plymouth, NC, and the county of Washington. The
property is accessed from US Hwy 64. It is approximately 1.5 miles west of the intersection of Hwy
64 and NC32.
Type of Development
The project includes the construction of a hotel and parking lot. The proposed roadway is also to be
developed at this time. At some future date the other four lots will be commercial developments.

from the center or high point into both ditches.

a. The proposed hotel site is to sheet flow across the parking lots into the curbing which directs
the water into the detention pond.
b. A storm drainage system has been designed to collect all future site development as well as
runoff from the roadway. The roadway has been designed to allow all runoff into this system.
c. A portion of the site water will drain in to the catch basin located at station 1+68.40 and then
be directed through the storm drainage system into the detention pond.
d. Future lots are to be graded to drain into proposed drainage system and all areas have been
used to determine sizing of the pond.
Pond Design
Using the entire project area and assuming 90% impervious on future sites and exact areas on the hotel
sites, I calculated 5.52 ac of impervious area. Using this data I calculated the following:

Monthly, or after every runoff producing rainfall event, whichever comes first:
A. Inspect the trash rack; remove accumulated debris, repair/replace if it is not functioning.
B. Inspect and clear the orifice of any obstructions. If a pump is used as the drawdown
mechanism, pump operation will be checked. A log of test runs of the pump will be kept on
site and made available to DWQ personnel upon request.
C. Inspect the pond side slopes and grassed inlet swales; remove trash, and repair eroded areas
before the next rainfall event.
D. If the pond is operated with a vegetated filter, the filter will be checked for sediment
accumulation, trash accumulation, erosion and proper operation of the flow spreader
mechanism. Repairs/cleaning will be done as necessary.

A. Inspect the collection system (ie. catch basins, piping, grassed swales) for proper functioning.
Accumulated trash will be cleared from basin grates, basin bottoms, and piping will be
checked for obstructions and cleared as required.
B. Pond inlet pipes will be checked for undercutting. Riprap or other energy dissipation
structures will be replaced, and broken pipes will be repaired.
III. Semi-annually (twice a year):
A. Accumulated sediment from the bottom of the outlet structure will be removed.

C. Grassed swales, including the vegetated filter if applicable, will be re -seeded twice a year as
necessary.

IV. Annually (Once a year):
A. The pond depth will be checked at various points. Sediment removal is required when the
depth is reduced to 75 % of the original design depth or 3', whichever is greater. Design
depth 5.5' measured vertically from the orifice down to the pond's bottom. Sediment must
be removed to at least the original design depth.
V. General:
A. Mowing of the side slopes will be accomplished according to the season. Maximum grass
height will be 6".
B. Cattails are encouraged along the pond perimeter, however they will be removed when they
cover more than 1/2 the surface area of the pond. The best time to cut them is at the end of the
growing season, in November. Environmentally sensitive chemicals for use in killing cattails
can also be used. Contact your nearest Agricultural Extension Agent.
C. The orifice/pump is designed to draw down the pond in 2-5 days. If drawdown is not
accomplished in that time, the orifice will be replaced with a larger or smaller orifice. Slow
drawdown may be attributed to a clogged system. The source of the clogging will be found and
eliminated.
D. All components of the detention pond system will be kept in good working order. Repair or
replacement components will meet the original design specifications as per the approved
stormwater plan. If previously approved components are determined to be ineffective, the
component must be redesigned and/or replaced.

Location of Project
This site is located within the city limits of Plymouth, NC, and the county of Washington. The
property is accessed from US Hwy 64. It is approximately L5 miles west of the intersection of Hwy
64 and NC32.
Type of Development
The project includes the construction of a hotel and parking lot. The proposed roadway is also to be
developed at this time. At some future date the other four lots will be commercial developments.

sites, I calculated 5.52 ac of impervious area Using this data I calculated the following:

Using the orifice equation I determined the size of the openings to have an outlet flow of 0.023cfs.

EROSION AND SEDIMENTATION CONTROL

Earth Disturbing Activities:
At this time the site has field grass growing. For site improvements the project will be stripped of the
top soil for placement of fill to the proposed grades. A drainage pond is to be constructed as well as
placement of storm drainage pipe. Also utilities will be extended onto the site including water and
sewer. After construction of the utilities the roadway will be graded, stoned, and paved. The additional
sites are to be graded to drain toward the proposed roadway.

B. EROSION AND SEDIMENTATION CONTROL MEASURES
1. A copy of the latest approved soil erosion and control plan must be on file at the job site. Timely
and proper attention shall be required for prevention of offsite erosion and sedimentation. Various
levels of erosion control are proposed in the plan. If it becomes evident that additional erosion and
sedimentation control measures are needed they shall be installed immediately. A permanent
ground cover, sufficient to restrain erosion, must be provided within the shorter of 15 working or
30 calendar days after completion of the construction or development.
2. A buffer zone, sufficient to restrain sedimentation, must be maintained between the land -
disturbing activity any adjacent property or water courses. Maintain / Do Not Disturb existing
vegetative cover in areas of no construction.
3. Install a gravel construction pad at the egress of US Hwy 64, as noted on the plans. The pad will
be a minimum of 50 It long, 30 ft. wide and 6" deep of washed 1-2" stone.
4. All disturbed areas will be seeded within 30 days following construction in accordance with the
seeding schedule as shown on the plans.
Rye grass is not to be used as ground cover.
5. Temporary sediment control primarily will consist of a silt fence along the northemly property
line to prevent silt from leaving the site at the exiting ditch. Also inlet protection around all storm
drainage will be used. The proposed site sheet drains to the detetion facility which will act as a
temporary sediment basin such to detain all sediments leaving the site until a substantial ground
cover is obtained. This pond is to be cleaned as needed after every major land disturbing rainfall
event.
C. TIME SEQUENCE OF OPERATIONS
Construction sequence is as noted on the sedimentation and erosion control plan sheet.
Construction is planned to begin October 1998.
