Staff review of the plans and specifications has determined that the protect, as proposed, will comply with the Stormwater Regulations set forth in Title 15A NCAC 2H.1000. This permit shall be effective from the date of issuance until November 13, 2019, and shall be subject to the following specified conditions and limitations: I. DESIGN STANDARDS This permit is effective only with respect to the nature and volume of stormwater described in the application and other supporting data. This stormwater system has been approved for the management of stormwater runoff as described in Section 1.6 on page 4 of this permit. The stormwater controls have been designed to handle the runoff from 1.04 acres of impervious area. All stormwater collection and treatment systems must be located in either dedicated common areas or recorded easements. The final plats for the project will be recorded showing all such required easements, in accordance with the approved plans and permit. 4. The tract will be limited to the amount of built -upon area indicated on page 4 of this permit, and per approved plans. Page 4 of 9 5. The runoff from all built -upon area within the permitted drainage areas of this project must be directed into the permitted stormwater control system. 6. The following design criteria have been provided for the infiltration basins and must be maintained at design condition: a. Total Site Area: 3.00 acres b. Total Impervious Surfaces: 1.04 acres C. Design Storm: • 1.5 - inch d. Basin Depth: 6.5 ft e. TSS removal efficiency: 90 % f. Permanent Pool Surface Area Required: 4010 sf g. Permanent Pool Surface Area Provided: 9785 sf h. Storage Volume Required: 6294 cf i. Storage Volume Provided: 13,091 cf j. Controlling Orifice: 3.50 ft k. Receiving Stream/River Basin: Sawyers Creek I. Classification of Water Body: C-sw II. SCHEDULE OF COMPLIANCE 1. The stormwater management system shall be constructed in its entirety, vegetated and operational for its intended use prior to the construction of any built -upon surface. 2. During construction, erosion shall be kept to a minimum and any eroded areas of the system will be repaired immediately. 3. The permittee shall at all times provide the operation and maintenance necessary to assure the permitted stormwater system functions at optimum efficiency. The approved Operation and Maintenance Plans must be followed in its entirety and maintenance must occur at the scheduled intervals including, but not limited to: a. Semiannual scheduled inspections (every 6 months). b. Sediment removal. C. Mowing and revegetation of slopes and the vegetated filter. d. Immediate repair of eroded areas. Page 5 of 9 e. Maintenance of all slopes in accordance with approved plans and specifications. f. Debris removal and unclogging of bypass structure, filter media, flow spreader, catch basins, piping and vegetated filter. g. A clear access path to the bypass structure must be available at all times. 4. Records of maintenance activities must be kept and made available upon request to authorized personnel of DWQ. The records will indicate the date, activity, name of person performing the work and what actions were taken. 5. The facilities shall be constructed as shown on the approved plans. This permit shall become voidable unless the facilities are constructed in accordance with the conditions of this permit, the approved plans and specifications, and other supporting data. 6. Upon completion of construction, prior to issuance of a Certificate of Occupancy, and prior to operation of this permitted facility, a certification must be received from an appropriate designer for the system installed certifying that the permitted facility has been installed in accordance with this permit, the approved plans and specifications, and other supporting documentation. Any deviations from the approved plans and specifications must be noted on the Certification. A modification may be required for those deviations. 7. If the stormwater system was used as an Erosion Control device, it must be restored to design condition prior to operation as a stormwater treatment device, and prior to occupancy of the facility. 8. Access to the stormwater facilities shall be maintained via appropriate easements at all times. 9. The permittee shall submit to the Director and shall have received approval for revised plans, specifications, and calculations prior to construction, for any modification to the approved plans, including, but not limited to, those listed below: a. Any revision to any item shown on the approved plans, including the stormwater management measures, built -upon area, details, etc. b. Project name change. C. Transfer of ownership. d. Redesign or addition to the approved amount of built -upon area or to the drainage area. e. Further subdivision, acquisition, lease or sale of all or part of the project area. The project area is defined as all property owned by the permittee, for which Sedimentation and Erosion Control Plan approval or a CAMA Major permit was sought. f. Filling in, altering, or piping of any vegetative conveyance shown on the approved plan. 10. The permittee shall submit final site layout and grading plans for any permitted future areas shown on the approved plans, prior to construction. Page 6 of 9 11. A copy of the approved plans and specifications shall be maintained on file by the Permittee at all times. 12. The Director may notify the permittee when the permitted site does not meet one or more of the minimum requirements of the permit. Within the time frame specified in the notice, the permittee shall submit a written time schedule to the Director for modifying the site to meet minimum requirements. The permittee shall provide copies of revised plans and certification in writing to the Director that the changes have been made. 13. This permit shall be effective from the date of issuance until November 13, 2019. Application for permit renewal shall be submitted 180 days prior to the expiration date of this permit and must be accompanied by the processing fee. III. GENERAL CONDITIONS This permit is not transferable except after notice to and approval by the Director. In the event of a change of ownership, or a name change, the permittee must submit a formal permit transfer request to the Division of Water Quality, accompanied by a completed name/ownership change form, documentation from the parties involved, and other supporting materials as may be appropriate. The approval of this request will be considered on its merits and may or may not be approved. The permittee is responsible for compliance with all permit conditions until such time as the Division approves the transfer request. 2. Failure to abide by the conditions and limitations contained in this permit may subject the Permittee to enforcement action by the Division of Water Quality, in accordance with North Carolina General Statute 143-215.6A to 143-215.6C. 3. The issuance of this permit does not preclude the Permittee from complying with any and all statutes, rules, regulations, or ordinances, which may be imposed by other government agencies (local, state, and federal) having jurisdiction. 4. In the event that the facilities fail to perform satisfactorily, including the creation of nuisance conditions, the Permittee shall take immediate corrective action, including those as may be required by this Division, such as the construction of additional or replacement stormwater management systems. 5. The permittee grants DENR Staff permission to enter the property during normal business hours for the purpose of inspecting all components of the permitted stormwater management facility. 6. The permit may be modified, revoked and reissued or terminated for cause. The filing of a request for a permit modification, revocation and reissuance or termination does not stay any permit condition. 7. Unless specified elsewhere, permanent seeding requirements for the stormwater control must follow the guidelines established in the North Carolina Erosion and Sediment Control Planning and Design Manual. Page 7 of 9 8. Approved plans and specifications for this project are incorporated by reference and are enforceable parts of the permit. A copy of the approved plans and specifications shall be maintained on file by the Permittee for the life of the project. 9. SUBMITTAL REQUIREMENTS Only complete application packages will be accepted and reviewed by the Division of Water Quality (DWQ). A complete package includes all of the items listed below. A detailed application instruction sheet and BMP checklists are available from http://h2o.enr.state.nc.us/su/bmp_forins.htm. The complete application package should be submitted to the appropriate DWQ Office. (The appropriate office may be found by locating project on the interactive online map at http://h2o.enr.state.nc.us/su/msi maps.htm.) Please indicate that the following required information have been provided by initialing in the space provided for each item. All original documents MUST be signed and initialed in blue ink. Download the latest versions for each submitted application package from http://h2o.enr.state.nc.us/su/bmp_forms.htm. 1. Original and one copy of the Stormwater Management Permit Application Form. 2. Original and one copy of the signed and notarized Deed Restrictions & Protective Covenants Form. (if required as per Part VII beloro) 3. If the receiving stream is reported as class SA or the receiving stream drains to class SA waters within 1h mile of the site boundary, include the 1h mile radius on the map. 7. Sealed, signed and dated calculations. 8. Two sets of plans folded to 8.5" x 14" (sealed, signed, & dated), including: a. Development/Project name. b. Engineer and firm. c. Copy of any applicable soils report with the associated SHWT elevations (Please identify elevations in addition to depths) as well as a map of the boring locations with the existing elevations and boring logs. Include an 8.5"xll" copy of the NRCS County Soils map with the project area clearly delineated. For projects with infiltration BMPs, the report should also include the soil type, expected infiltration rate, and the method of determining the infiltration rate. For corporations and limited liability corporations (LLC): Provide documentation from the NC Secretary of State or other official documentation, which supports the titles and positions held by the persons listed in Contact Information, item la, 2a, and/or 3a per NCAC 2H.1003(e). If lot sizes vary significantly or the proposed BUA allocations vary, a table listing each lot number, lot size, and the allowable built -upon area must be provided as an attachment to the completed and notarized deed restriction form. The appropriate deed restrictions and protective covenants forms can be downloaded from http://h2o.enr.state.nc.us/su/bmp forms.htmtldeed restrictions. Download the latest versions for each submittal. In the instances where the applicant is different than the property owner, it is the responsibility of the property owner to sign the deed restrictions and protective covenants form while the applicant is responsible for ensuring that the deed restrictions are recorded. By the notarized signature(s) below, the permit holder(s) certify that the recorded property restrictions and protective covenants for this project, if required, shall include all the items required in the permit and listed on the forms available on the website, that the covenants will be binding on all parties and persons claiming under them, that they will run with the land, that the required covenants cannot be changed or deleted without concurrence from the NC DWQ, and that they will be recorded prior to the sale of any lot. The site is Lot 1 of the Minor Commercial Subdivision for Russell Twiford and is zoned Highway Commercial. The site is a mostly cleared, old home site. Some trees located around building remnants are to be removed and surcharge fill is to be placed on the site within the next week. This work will be performed in accordance with the Letter of Approval with Modifications for Erosion and Sedimentation Control Plan No. Camde-2010-002. The site will be developed into primary office building complex for the credit union with two entrances along Rosa belle Court and a third entrance along US-158. Development will consist of constructing a building that will cover 5,720 sf (with a potential for a future addition of 1547 sf), 2,863 sf of sidewalk, and 38,671 sf of paved parking and drive aisles. All runoff from impervious surfaces will be collected in an underground collection system and routed to a wet detention basin. The new detention pond will meet state requirements for 90% TSS removal and storage of the first 1.5" of runoff as well as the county's requirement of limiting post development runoff to pre development levels during a 10-year storm event. The pond will discharge into the existing side yard ditch on the southern property line that ultimately drains into the wetland area some 500 feet northwest from the western property corner. According to the Web Soil Survey, the site has three different soil types. About half of the lot is Augusta, while the front quarter is Tomotley and the rear quarter is State. A soils evaluation dated December 22, 2008 prepared by Soil & Environmental Consultants, PA has been attached. The report includes a quad map, soils map, and boring logs. The evaluation for the detention pond was performed in the left rear corner of the site where the State soils are located. According to the report, the seasonal high water table is less than 12-24" below existing grade. With an average grade of 5', the seasonal high water table is estimated to be between elevations 3 and 4. The high water table and low permeability rates make wet detention the most appropriate form of storm water treatment. The permanent pool elevation has been set at 3.5'. Drainage Area 1 contains 1.84 acres and encompasses a majority of the site including all proposed on -site impervious except for small portions of the driveways as they connect to the adjacent roads. This drainage area will contain the entire building, 35,747 sf of paved parking lot, and all the concrete sidewalk. The total impervious area will be 45,364 sf or 56.56% of the drainage area. Runoff from the building will be collected in a French drain system and routed through the underground pipe system. The runoff from the parking lot will sheet flow across the parking lot to various inlets. It will then be routed through the same underground piping system as the building. The pipe system has been designed to drain to the detention pond. Drainage Area 2 contains the remaining 1.16 acres of the site and 1,628 sf of off -site area which includes the pervious areas around the perimeter of the site as well as portions of the three driveways. Dear Mr Potratz: Soil & Environmental Consultants, PA (S&EC) performed a detailed soil evaluation at the site mentioned above. A map was provided by the architect showing the location of the proposed wet detention pond and where soil borings should be performed by S&EC. At the indicated locations the following were performed. a profile description to seven feet (or as deep as possible with hand augers), estimation of permeability, hydraulic conductivity, and seasonal high water table based on soil color, and apparent (observed) water table measurement (groundwater rise in auger holes). The purpose of this evaluation was to provide you with additional information for designing a stormwater wet detention pond. The following is a brief report of the methods utilized in this evaluation and the results obtained. Soil/Site Evaluation & Results A site evaluation was performed by Walter Cole and Sarah Menser (Licensed Soil Scientist and SSIT) to acquire the information requested. The evaluation was performed by making hand auger borings at several locations which were located with a GPS receiver and are shown on the attached map and identifying and recording the soil morphological conditions at those locations in order to develop soil profile descriptions, which are also included with this report and shown on the attached map. Seasonal Hieh Water Table Estimation We were able to directly measure the observed (apparent) water table at all of the boring locations during the time of our evaluation, and also base the soil wetness on low chroma (<2) colors in the Munsell Soil Color Chart. The seasonal high water table based on soil color ranged from less than 12" — 24 inches from natural ground surface for all borings, and the apparent (observed) water table at one hour was 12 inches from natural ground level, in all of the auger holes, however, please keep in mind this is just a "snap -shot' measurement, the reality is that the actual seasonal high water table is most likely deeper than what is observed by soil color, especially with adjacent existing agricultural ditching, and deeper than was is observed by apparent water table due to recent rains and the perched water table "pushing" water up into the auger holes. The soil conditions on -site seem favorable for a wet detention pond with respect to seasonal high water table, however the groundwater lowering effects of the existing agricultural drainage, and the coarse loamy upper soil horizons will have to be taken into account when deciding weather or not to incorporate a liner into the design. Permeabilitv/hydraulic Conductivity Estimation Most soil borings revealed a sandy loam surface horizon over a sandy clay loam subsoil with underlying marine clay parent material. The clay subsurface horizon at all locations was massive with respect to soil structure and extremely firm, very sticky, very plastic with respect to soil consistency These are soil characteristics that are indicators of expansive clay mineralogy, which forms very slowly permeable soils with estimated hydraulic conductivity ranges from .01- 14 inches/hour. The soil conditions on -site seem somewhat favorable for a wet detention pond with respect to soil permeability/hydraulic conductivity as far- as the marine clay susbsurface horizons, however the shallower coarse loamy upper soil horizons will probably require the use of a liner to maintain water levels inside the wet pond. Please refer to the enclosed map and soil profile descriptions for more information. All of these estimates obtained from one or more of the following: Camden County Soil Survey "Physical & Chemical Properties of the Soil Permeability"; Augusta, Tomotley, Roanoke, Sununerfield Soil Series and/or USDA-NRCS (United States Department of Agriculture - Natural Resource Conservation Service) official series description, and/or "NRCS Field Book for Describing & Sampling Soils" the permeability estimates were converted to conductivity on the profile description tables. No actual site specific testing of hydraulic conductivity was performed, however this service can be provided by our firm if requested.
