Stormwater Permit SW7090325 for a stormwater treatment system consisting of a Sand
Filter System serving a Family Karate Center located on US Hwy 70 in Havelock, NC
expires on December 30, 2021. This is a reminder that permit renewal applications are
due 180 days prior to expiration. We do not have a record of receiving a renewal
application.
Please submit a completed permit renewal application along with a $505.00 fee for
permit renewal. Permit application forms for renewal can be found on our website at:

Statutes and the Coastal Stormwater rules require that this property be covered under a

Stormwater Management Permit SW7090325
Family Karate Center

Dear Mr. Williams:
On September 14, 2016 a stormwater compliance inspection was conducted at a facility that was
permitted as the Family Karate Center located on US HWY 70 East in Havelock, NC. State Stormwater
permit SW7090325 was issued on February 3, 2012 for the construction and operation of a sand filter
system to treat stormwater runoff from impervious surfaces on the site. The sand filter system was found
to be in need of maintenance. Vegetation was growing over a significant portion of the sand filter surface.
Please remove the vegetation, check the depth of sediment in the sediment chamber and return the sand
filter to its original operating condition within 30 days and notify this office when complete so that the
facility can be re -inspected.
Permit SW7090325 requires that this stormwater treatment system be properly maintained as designed.
Enclosed is a copy of the operation and maintenance agreement signed by you during the permitting
process. Failure to comply with a permit subjects the owner to assessment of civil penalties.

The 1-1/2" will be controlled and treated by a sedimentation chamber/sand filter/grass filter. All impervious
surfaces will either sheet flow and drain into catch basins and into the sedimentation chamber, over the weir,
across entire length of sand filter and filter through sand and drain out to existing swale alond service road. The
sand filter is concrete, with a bottom elevation 1' above seasonal high water table.

permitted in July 2009 by your office. Heather Steinecher was the reviewer and the site
was permitted as High Density with a closed sand filter and sediment chamber forebay
facility within the City of Havelock, NC limits. The owner is now ready to start
construction, however there have been a few changes, primarily reduction of impervious
surfaces within site.
We are submitting this package as a permit modification. Enclosed are revised drawings,
original submission documentation, Stormwater Permit SW7090325 and revised forms,
including Sandfilter Supplement and Stormwater Application and check to NC DENR for
$505.

STORMWATER PERMIT MODIFICATION
NARRATIVE
Introduction and Existing Conditions
This narrative discusses modification of stormwater permit SW7090325 (August 31, 2009) for the
Family Karate Center project. Originally the site plan proposed construction of the Family Karate
Center project and 18,052 sq.ft of impervious surfaces, including a 7,000 sqft commercial facility.
Total impervious surfaces have been reduced to 14,840 sqft by a reduction in paved parking area.
Building to remain 7,000 sq.ft.. The site is located on the south side of the service road of US Highway
70E, within the City of Havelock, Craven County, North Carolina. The easterly adjacent parcel is
currently wooded and vacant. The westerly adjacent parcel is Awake Insurance agency. The plan is for
construction of a slab on -grade, metal -building, finish floor is 49.6 feet. The total lot is 0.4789 acre and
is zoned commercial. The owner and operator of the facility is Mr. Frank Williams, PhD.
In general, the parcel drains over a minimal grade towards an existing ditch along the service road. As
seen on the included location and topo map, the ditch and the site drains to an unclassified existing
ditch, then over 1 1/2 miles to Slocum Creek a tributary of the Neuse River, classified as a SC, SW and
NSW water.
STORMWATER MANAGEMENT
The entire site is less than %2 ac of disturbed area, as such no erosion and sediment control permit is
required. However, located in a coastal county, a stormwater facility is required to manage the first 1.5
inches of stormwater as the proposed project disturbs the entire site (over 10,000 sqft) of
buildable/impervious surfaces -driveway and building. As seen on the attached drawings, the required
stormwater event (1.5") is being stored and treated by a sediment forebay and sand filter. All
impervious surfaces (parking area and roof drains) will drain to the catch basins 95 feet by 4.25 feet and
overland flow to sediment forebay. Run off will settle and flow across the weir across the full length of
the sand filter (95 feet by 4.65 feet). Runoff will filter into and through the sand and into a slotted
drain, outfalling to the existing ditch along the service road. Events greater than the required volume
will discharge to a grassed filter and then into the existing ditch.

The proposed design is for 0.47 total acre site for 7,000 sqft facility. The site includes 14,840 sqft of
proposed impervious surfaces, building and parking. All run off will drain to the sediment forebay and
sand filter.
The area of impervious surfaces has been reduced from the permitted area by over 3,200 sq.ft. The
revised sandfilter/sediment forebay are sized to contain the required event, draw down, freeboard,
meeting or exceeding all DENR size, depth and volume requirements.

The Washington Regional Office received a complete Stormwater Management Permit Application for
the Family Karate Center on July 10, 2009. Staff review of the plans and specifications has
determined that the project, as proposed, will comply with the Stormwater Regulations set forth in
Session Law 2008-211 and Title 15A NCAC 2H.1000. We are forwarding Permit No. SW7090325
dated August 31, 2009, for the construction of the subject project.
This permit shall be effective from the date of issuance until August 31, 2019, and shall be subject to
the conditions and limitations as specified therein. Please pay special attention to the Operation and
Maintenance requirements in this permit. Failure to establish an adequate system for operation and
maintenance of the stormwater management system will result in future compliance problems.
If any parts, requirements, or limitations contained in this permit are unacceptable, you have the right
to request an adtudicatory, hearing upon written request within sixty (60) days following receipt of this
permit. This request must be in the form of a written petition, conforming to Chapter 150E of the

construction, operation and maintenance of one closed sand filter(s) in compliance with the
provisions of Session Law 2008-211 and 15A NCAC 2H .1000 (hereafter referred to as the
stormwater rules') and the approved stormwater management plans and specifications
and other supporting data as attached and on file with and approved by the Division of
Water Quality and considered a part of this permit.
This permit shall be effective from the date of issuance until August 31, 2019, and shall be
subject to the following specified conditions and limitations:
I. DESIGN STANDARDS
This permit is effective only with respect to the nature and volume of stormwater
described in the application and other supporting data.
This stormwater system has been approved for the management of stormwater
runoff as described in Section 1.8 on page 3 of this permit. The stormwater control
has been designed to handle the runoff from 18,052 square feet of impervious area.
3. This basin must be operated with a 30 foot vegetated grass filter.
4. The tract will be limited to the amount of built -upon area indicated in Section 1.8 of
this permit, and as shown on the approved plans.
5. All stormwater collection and treatment systems must be located in either dedicated
common areas or recorded easements. The final plats for the project will be
recorded showing all such required easements, in accordance with the approved
plans.
The runoff from all built -upon area within the permitted drainage area of this project
must be directed into the permitted stormwater control system.
The built -upon areas associated with this project shall be located at lest 50 feet
landward of all surface waters.

The stormwater management system shall be constructed in its entirety, vegetated
and operational for its intended use prior to the construction of any built -upon
surface.
2. During construction, erosion,shall be kept to a minimum and any eroded areas of
the system will be repaired immediately.
3. The permittee shall at all times provide the operation and maintenance necessary to
assure the permitted stormwater system functions at optimum efficiency. The
approved Operation and Maintenance Plan must be followed in its entirety and
maintenance must occur at the scheduled intervals including, but not limited to:
a. Semiannual scheduled inspections (every 6 months).
b. Sediment removal.
C. Mowing and revegetation of slopes and the vegetated filter.
d. Immediate repair of eroded areas.
e. Maintenance of all slopes in accordance with approved plans and
specifications.
f. Debris removal and unclogging of bypass structure, infiltration media, flow
spreader, catch basins, piping and vegetated filter.
g. A clear access path to the bypass structure must be available at all times.
4. Records of maintenance activities must be kept for each permitted BMP. The
reports will indicate the date, activity, name of person performing the work and what
actions were taken.
5. The permittee shall submit to the Division of Water Quality an annual summary
report of the maintenance and inspection records for each BMP. The report shall
summarize the inspection dates, results of the inspections, and the maintenance
work performed at each inspection.
6. The facilities shall be constructed as shown on the approved plans. This permit shall
become voidable unless the facilities are constructed in accordance with the
conditions of this permit, the approved plans and specifications, and other
supporting data.
If the stormwater system was used as an Erosion Control device, it must be
restored to design condition prior to operation as a stormwater treatment device,
and prior to occupancy of the facility.

8. Upon completion of construction, prior to issuance of a Certificate of Occupancy,
and prior to operation of this permitted facility, a certification must be received from
an appropriate designer for the system installed certifying that the permitted facility
has been installed in accordance with this permit, the approved plans and
specifications, and other supporting documentation. Any deviations from the
approved plans and specifications must be noted on the Certification. A modification
may be required for those deviations.
9. Access to the stormwater facilities shall be maintained via appropriate easements at
all times.
10. The permittee shall submit to the Director and shall have received approval for
revised plans, specifications, and calculations prior to construction, for any
modification to the approved plans, including, but not limited to, those listed below:
a. Any revision to any item shown on the approved plans, including the
stormwater management measures, built -upon area, details, etc.
b. Project name change.
C. Transfer of ownership.
d. Redesign or addition to the approved amount of built -upon area or to the
drainage area.
e. Further subdivision, acquisition, lease or sale of all or part of the project area.
The project area is defined as all property owned by the permittee, for which
Sedimentation and Erosion Control Plan approval or a CAMA Major permit
was sought.
f. Filling in, altering, or piping of any vegetative conveyance shown on the
approved plan.
11. The permittee shall submit final site layout and grading plans for any permitted
future areas shown on the approved plans, prior to construction.
12. A copy of the approved plans and specifications shall be maintained on file by the
Permittee at all times.
13. The Director may notify the permittee when the permitted site does not meet one or
more of the minimum requirements of the permit. Within the time frame specified in
the notice, the permittee shall submit a written time schedule to the Director for
modifying the site to meet minimum requirements. The permittee shall provide
copies of revised plans and certification in writing to the Director that the changes
have been made.
III. GENERAL CONDITIONS
This permit is not transferable to any person or entity except after notice to and
approval by the Director. In the event of a change of ownership, or a name change,
the permittee must submit a completed Name/Ownership Change form to the
Division of Water Quality, signed by both parties, and accompanied by the
appropriate documentation as listed on page 2 of the form. The approval of this
request will be considered on its merits and may or may not be approved.
The permittee is responsible for compliance with all permit conditions until such time
as the Division approves any transfer request.
Failure to abide by the conditions and limitations contained in this permit may
subject the Permittee to enforcement action by the Division of Water Quality, in
accordance with North Carolina General Statute 143-215.6A to 143-215.6C.
The issuance of this permit does not preclude the Permittee from complying with
any and all statutes, rules, regulations, or ordinances, which may be imposed by
other government agencies (local, state, and federal) having jurisdiction.

5. In the event that the facilities fail to perform satisfactorily, including the creation of
nuisance conditions, the Permittee shall take immediate corrective action, including
those as may be required by this Division, such as the construction of additional or
replacement stormwater management systems.
6. The permittee grants DENR Staff permission to enter the property during normal
business hours for the purpose of inspecting all components of the permitted
stormwater management facility.
7. The permit issued shall continue in force and effect until revoked or terminated. The
permit may be modified, revoked and reissued or terminated for cause. The filing of
a request for a permit modification, revocation and reissuance or termination does
not stay any permit condition.
8. Unless specified elsewhere, permanent seeding requirements for the stormwater
controls must follow the guidelines established in the North Carolina Erosion and
Sediment Control Planning and Design Manual.
9. Approved plans and specifications for this project are incorporated by reference and
are enforceable parts of the permit.
10. The issuance of this permit does not prohibit the Director from reopening and
modifying the permit, revoking and reissuing the permit, or terminating the permit as
allowed by the laws, rules and regulations contained in Session Law 2008-211, Title
15A NCAC 2H.1000, and NCGS 143-215.1 et. al.
11. The permittee shall notify the division in writing of any name, ownership or mailing
address changes at least 30 days prior to making such changes.
12. This permit shall be effective from the date of issuance until August 31, 2019.
Application for permit renewal shall be submitted 180 days prior to the expiration
date of this permit and must be accompanied by the processing fee.

provisions of Session Law 2008-211 and 15A NCAC 2H .1000 (hereafter referred to as the
"stormwater rules') and the approved stormwater management plans and specifications
and other supporting data as attached and on file with and approved by the Division of
Water Quality and considered a part of this permit.
This permit shall be effective from the date of issuance until August 31, 2019, and shall be
subject to the following specified conditions and limitations:
I. DESIGN STANDARDS
This permit is effective only with respect to the nature and volume of stormwater
described in the application and other supporting data.
2. This stormwater system has been approved for the management of stormwater
runoff as described in Section 1.8 on page 3 of this permit. The stormwater control
has been designed to handle the runoff from 18,052 square feet of impervious area.
This basin must be operated with a 30 foot vegetated grass filter.
4. The tract will be limited to the amount of built -upon area indicated in Section 1.8 of
this permit, and as shown on the approved plans.
All stormwater collection and treatment systems must be located in either dedicated
common areas or recorded easements. The final plats for the project will be
recorded showing all such required easements, in accordance with the approved
plans.
6. The runoff from all built -upon area within the permitted drainage area of this project
must be directed into the permitted stormwater control system.
7. The built -upon areas associated with this project shall be located at lest 50 feet
landward of all surface waters.

The stormwater management system shall be constructed in its entirety, vegetated
and operational for its intended use prior to the construction of any built -upon
surface.
2. During construction, erosion shall be kept to a minimum and any eroded areas of
the system will be repaired immediately.
3. The permittee shall at all times provide the operation and maintenance necessary to
assure the permitted stormwater system functions at optimum efficiency. The
approved Operation and Maintenance Plan must be followed in its entirety and
maintenance must occur at the scheduled intervals including, but not limited to:
a. Semiannual scheduled inspections (every 6 months).
b. Sediment removal.
C. Mowing and revegetation of slopes and the vegetated filter.
d. Immediate repair of eroded areas.
e. Maintenance of all slopes in accordance with approved plans and
specifications.
f. Debris removal and unclogging of bypass structure, infiltration media, flow
spreader, catch basins, piping and vegetated filter.
g. A clear access path to the bypass structure must be available at all times.
4. Records of maintenance activities must be kept for each permitted BMP. The
reports will indicate the date, activity, name of person performing the work and what
actions were taken.
5. The permittee shall submit to the Division of Water Quality an annual summary
report of the maintenance and inspection records for each BMP. The report shall
summarize the inspection dates, results of the inspections, and the maintenance
work performed at each inspection.
6. The facilities shall be constructed as shown on the approved plans. This permit shall
become voidable unless the facilities are constructed in accordance with the
conditions of this permit, the approved plans and specifications, and other
supporting data.

8. Upon completion of construction, prior to issuance of a Certificate of Occupancy,
and prior to operation of this permitted facility, a certification must be received from
an appropriate designer for the system installed certifying that the permitted facility
has been installed in accordance with this permit, the approved plans and
specifications, and other supporting documentation. Any deviations from the
approved plans and specifications must be noted on the Certification. A modification
may be required for those deviations.
9. Access to the stormwater facilities shall be maintained via appropriate easements at
all times.
10. The permittee shall submit to the Director and shall have received approval for
revised plans, specifications, and calculations prior to construction, for any
modification to the approved plans, including, but not limited to, those listed below:
a. Any revision to any item shown on the approved plans, including the
stormwater management measures, built -upon area, details, etc.
b. Project name change.
C. Transfer of ownership.
d. Redesign or addition to the approved amount of built -upon area or to the
drainage area.
e. Further subdivision, acquisition, lease or sale of all or part of the project area.
The project area is defined as all property owned by the permittee, for which
Sedimentation and Erosion Control Plan approval or a CAMA Major permit
was sought.
f. Filling in, altering, or piping of any vegetative conveyance shown on the
approved plan.
11. The permittee shall submit final site layout and grading plans for any permitted
future areas shown on the approved plans, prior to construction.
12. A copy of the approved plans and. specifications shall be maintained on file by the
Permittee at all times.
13. The Director may notify the permittee when the permitted site does not meet one or
more of the minimum requirements of the permit. Within the time frame specified in
the notice, the permittee shall submit a written time schedule to the Director for
modifying the site to meet minimum requirements. The permittee shall provide
copies of revised plans and certification in writing to the Director that the changes
have been made.
III. GENERAL CONDITIONS
This permit is not transferable to any person or entity except after notice to and
approval by the Director. In the event of a change of ownership, or a name change,
the permittee must submit a completed Name/Ownership Change form to the
Division of Water Quality, signed by both parties, and accompanied by the
appropriate documentation as listed on page 2 of the form. The approval of this
request will be considered on its merits and may or may not be approved.
2. The permittee is responsible for compliance with all permit conditions until such time
as the Division approves any transfer request.
3. Failure to abide by the conditions and limitations contained in this permit may
subject the Permittee to enforcement action by the Division of Water Quality, in
accordance with North Carolina General Statute 143-215.6A to 143-215.6C.
4. The issuance of this permit does not preclude the Permittee from complying with
any and all statutes, rules, regulations, or ordinances, which may be imposed by
other government agencies (local, state, and federal) having jurisdiction.

5. In the event that the facilities fail to perform satisfactorily, including the creation of
nuisance conditions, the Permittee shall take immediate corrective action, including
those as may be required by this Division, such as the construction of additional or
replacement stormwater management systems.
6. The permittee grants DENR Staff permission to enter the property during normal
business hours for the purpose of inspecting all components of the permitted
stormwater management facility.
7. The permit issued shall continue in force and effect until revoked orterminated. The
permit may be modified, revoked and reissued or terminated for cause. The filing of
a request for a permit modification, revocation and reissuance or termination does
not stay any permit condition.
8. Unless specified elsewhere, permanent seeding requirements for the stormwater
controls must follow the guidelines established in the North Carolina Erosion and
Sediment Control Planning and Design Manual.
9. Approved plans and specifications for this project are incorporated by reference and
are enforceable parts of the permit.
10. The issuance of this permit does not prohibit the Director from reopening and
modifying the permit, revoking and reissuing the permit, or terminating the permit as
allowed by the laws, rules and regulations contained in Session Law 2008-211, Title
15A NCAC 21-1.1000, and NCGS 143-215.1 et. al.
11. The permittee shall notify the Division in writing of any name, ownership or mailing
address changes at least 30 days prior to making such changes.
12. This permit shall be effective from the date of issuance until August 31, 2019.
Application for permit renewal shall be submitted 180 days prior to the expiration
date of this permit and must be accompanied by the processing fee.

INTRODUCTION
The following report contains the results of subsurface field test, to establish the in situ height of
the seasonal high water table. Neither this report, nor the field tests performed are intended to
serve as geotechnical recommendations or as a basis of pavement design selection. On March
03, 2009 KSMJ, pllc performed a subsurface soil exploration at the site of the proposed Family
Karate Center project. The site is to be located at 473 US Highway 70, approximately two miles
west of State Road 101, within the City limits of Havelock, Craven County.
Based on information from the provided plans, a sedimentation chamber and sand filter system is
proposed to meet the DENR high density stormwater requirements. As such, hand auger borings
were performed at 50 foot intervals 25 feet from the southerly property line. From general
observations of the existing and historical use of the property it appears the property has been
been vacant for some time. Per the Craven County soils map, the in -situ soils are of the Rain
series. Based on the observations and soil classifications, the generalized subsurface conditions,
the depth to seasonal highwater table appears to be approximately 2.8-3 feet.

STORMWATER PERMIT NARRATIVE
Introduction and Existing Conditions
This narrative discusses the high density stormwater permit application for the Family Karate Center
project. This site plan proposes construction of a Family Karate Center project, including a 7,000 sqft
commercial facility, located on the south side of the service road of US Highway 70E, within the City
of Havelock, Craven County, North Carolina. The easterly adjacent parcel is currently wooded and
vacant. The westerly adjacent parcel is Awake Insurance agency. The plan is for construction of a slab
on -grade, metal -building, finish floor is 50.5 feet. The building will include two office/commercial
facilities units (1,200 sqft each) and an indoor recreation Karate Center 3,200 sqft and 1,400 sqft
storage. The proposed project includes a total of 18,052 sqft (over 10,000 sqft) of buildable/impervious
surfaces -driveway and building and 20,861 sq.ft. of disturbance. The total lot is 0.478 acre and is zoned
commercial. The owner and operator of the facility is Mr. Frank Williams, PhD.
In general, the parcel drains over a minimal grade southerly towards an existing ditch along the service
road. As seen on the included location and topo map, the ditch and the site drains to an unclassified
existing ditch, then over I1/2 miles to Slocum Creek a tributary of the Neuse River, classified as a SC,
SW and NSW water.
STORMWATER MANAGEMENT
The site is less than and includes less than %, ac of disturbed area, as such no erosion and sediment
control permit is required. However, located in a coastal county, a stormwater facility is now required
to manage and treat the first 1.5 inches of stormwater. As seen on the attached drawings, the required
stormwater event (1.5") is being managed by a sedimentation chamber and sand filter stormwater
system. All impervious surfaces (driveway and roof drains) will drain to the 100 feet by 3.55 feet by
3.5 feet sand filter/sedimentation chamber located along the rear of the parcel. The filter will outfall to
a 30 foot grass filter and into the existing ditch along the service road.

CONCLUSION
The proposed design is for 0.47 total acre site for a 7,000 sgft facility. The site includes 18,052 sgft of
proposed impervious surfaces, building and parking. All run off will drain to the sedimentation
chamber and sand filter, grass filter and into an existing drainage Swale. Additionally, the proposed
facility contains the required event, draw down, max head, meeting or exceeding all DENR size, depth
and volume requirements.

Thank you for your help with this project. We received your review and have addressed
your concerns for final submission, as follows:
Comment 1: The seasonally high groundwater table must be at least two feet below the
bottom of the filter for open -bottom designs.
Response 1: As discussed in the narrative, this is a small site, less than one half of an
acre. Per the site conditions, it is very difficult to meet the each of design requirements
of ANY of the stormwater filter devices. After reviewing the existing site conditions
and various bmp options, it was determined that the proposed sand filter is the only
BMP device that meets all of the site conditions and DENR requirements, except
SHWT separation requirement, with only one foot of separation. We designed the sand
filter with an impervious bottom and side layers to ensure there is no filtration into the
water table. As such, the 1-112" will be controlled and treated through the
sedimentation chamber and sand filter.
Comment 2: In calculating the Design Volume (WQV) it appears the Impervious area
was used in the calculation. The entire Drainage Area must be used on computing this
Volume. Please revise accordingly.

STORMWATER PERMIT NARRATIVE
Introduction and Existing Conditions
This narrative discusses the high density stormwater permit application for the Family Karate Center
project. This site plan proposes construction of a Family Karate Center project, including a 7,000 sqft
commercial facility, located on the south side of the service road of US Highway 70E, within the City
of Havelock, Craven County, North Carolina. The easterly adjacent parcel is currently wooded and
vacant. The westerly adjacent parcel is Awake Insurance agency. The plan is for construction of a
slab on -grade, metal -building, finish floor is 50.5 feet. The building will include two
office/commercial facilities units (1,200 sqft each) and an indoor recreation Karate Center 3,200 sqft
and 1,400 sqft storage. The proposed project includes a total of 18,052 sqft (over 10,000 sqft) of
buildable/impervious surfaces -driveway and building and 20,861 sq.ft. of disturbance. The total lot is
0.478 acre and is zoned commercial. The owner and operator of the facility is Mr. Frank Williams,
PhD.
In general, the parcel drains over a minimal grade southerly towards an existing ditch along the
service road. As seen on the included location and topo map, the ditch and the site drains to an
unclassified existing ditch, then over 11/2 miles to Slocum Creek a tributary of the Neuse River,
classified as a SC, SW and NSW water.
STORMWATER MANAGEMENT
The site is less than and includes less than '/z ac of disturbed area, as such no erosion and
sediment control permit is required. However, located in a coastal county, a stormwater
facility is now required to manage and treat the first 1.5 inches of stormwater.

As seen on the attached drawings, the required stormwater event (1.5") is being managed by
a sedimentation chamber and sand filter stormwater system. All impervious surfaces
(driveway and roof drains) will drain to the 102 feet by 4.36 feet by 3.67 feet sand
filter/sedimentation chamber located along the rear of the parcel. The filter will outfall to a
30 foot grass filter and into the existing ditch along the service road.

CONCLUSION
The proposed design is for 0.47 total acre site for a 7,000 sqft facility. The site includes 18,052 sqft
of proposed impervious surfaces, building and parking. All run off will drain to the sedimentation
chamber and sand filter, grass filter and into an existing drainage swale. Additionally, the proposed
facility contains the required event, draw down, max head, meeting or exceeding all DENR size,
depth and volume requirements.

In accordance with Title 15 NCAC 2H.1000, the Coastal Stormwater Management Regulations, deed
restrictions and protective covenants are required for High Density Residential Subdivisions where
lots will be subdivided and sold and runoff will be treated in an engineered stormwater control facility.
Deed restrictions and protective covenants are necessary to ensure that the development maintains a
"built -upon" area consistent with the design criteria used to size the stormwater control facility.

1. The following covenants are intended to ensure ongoing compliance with State Stormwater

STORMWATER PERMIT NARRATIVE
Introduction and Eaistine Conditions
This narrative discusses the high density stormwater permit application for the Family Karate Center
project. This site plan proposes construction of a Family Karate Center project, including a 7,000 sqft
commercial facility, located on the south side of the service road of US Highway 70E, within the City
of Havelock, Craven County, North Carolina. The easterly adjacent parcel is currently wooded and
vacant. The westerly adjacent parcel is Awake Insurance agency. The plan is for construction of a slab
on -grade, metal -building, finish floor is 50.5 feet. The building will include two office/commercial
facilities units (1,200 sqft each) and an indoor recreation Karate Center 3,200 sqft and 1,400 sqft
storage. The proposed project includes a total of 18,052 sqft (over 10,000 sqft) of buildable/impervious
surfaces -driveway and building and 20,861 sq.ft. of disturbance. The total lot is 0.478 acre and is zoned
commercial. The owner and operator of the facility is Mr. Frank Williams, PhD.
In general, the parcel drains over a minimal grade southerly towards an existing ditch along the service
road. As seen on the included location and topo map, the ditch and the site drains to an unclassified
existing ditch, then over 11/2 miles to Slocum Creek a tributary of the Neuse River, classified as a SC,
SW and NSW water.
STORMWATER MANAGEMENT
The site is less than and includes less than '/2 ac of disturbed area, as such no erosion and sediment
control permit is required. However, located in a coastal county, a stormwater facility is now required
to manage and treat the first 1.5 inches of stormwater. As seen on the attached drawings, the required
stormwater event (1.5") is being managed by a sedimentation chamber and sand filter stormwater
system. All impervious surfaces (driveway and roof drains) will drain to the 100 feet by 3.55 feet by
3.5 feet sand filter/sedimentation chamber located along the rear of the parcel. The filter will outfall to
a 30 foot grass filter and into the existing ditch along the service road.

Sand Filter Operation and Maintenance Agreement
I will keep a maintenance record on this BMP. This maintenance record will be kept in a
log in a known set location. Any deficient BMP elements noted in the inspection will be
corrected, repaired or replaced immediately. These deficiencies can affect the integrity of
structures, safety of the public, and the removal efficiency of the BMP.

In accordance with Title 15 NCAC 21-1.1000, the Coastal Stormwater Management Regulations, deed
restrictions and protective covenants are required for High Density Residential Subdivisions where
lots will be subdivided and sold and runoff will be treated in an engineered stormwater control facility.
Deed restrictions and protective covenants are necessary to ensure that the development maintains a
"built -upon" area consistent with the design criteria used to size the stormwater control facility.

1. The following covenants are intended to ensure ongoing compliance with State Stormwater
Management Permit Number as issued by the Division of Water Quality

INTRODUCTION
The following report contains the results of subsurface field test, to establish the in situ height of
the seasonal high water table. Neither this report, nor the field tests performed are intended to
serve as geotechnical recommendations or as a basis of pavement design selection. On March
03, 2009 KSMJ, pllc performed a subsurface soil exploration at the site of the proposed Family
Karate Center project. The site is to be located at 473 US Highway 70, approximately two miles
west of State Road 101, within the City limits of Havelock, Craven County.
Based on information from the provided plans, a sedimentation chamber and sand filter system is
proposed to meet the DENR high density stormwater requirements. As such, hand auger borings
were performed at 50 foot intervals 25 feet from the southerly property line. From general
observations of the existing and historical use of the property it appears the property has been
been vacant for some time. Per the Craven County soils map, the in -situ soils are of the Rain
series. Based on the observations and soil classifications, the generalized subsurface conditions,
the depth to seasonal highwater table appears to be approximately 2.8-3 feet.
FIELD EXPLORATION

Topsoil to 6 inches, friable with coarse roots.
Beneath top soil was grayish -brown and sandy loam, with fine roots.
Groundwater table was encountered at 2.8 to 3 feet, elevation 45.2

foot right-of-way of the Service Road designated as NCSR #1774,
said point iron being located South 24-23 East 993.26 feet, South
70-40 West 30.13 feet and South 24-35 East 104.79 feet from a
P.K. nail at the centerline intersection of the aforesaid Service Road

Subject: Subsurface soil exploration
Family Karate Center
INTRODUCTION
The following report contains the results of subsurface field test, to establish the in situ height of
the seasonal high water table. Neither this report, nor the field tests performed are intended to
serve as geotechnical recommendations or as a basis of pavement design selection. On March
03, 2009 KSMJ, pllc performed a subsurface soil exploration at the site of the proposed Family
Karate Center project. The site is to be located at 473 US Highway 70, approximately two miles
west of State Road 101, within the City limits of Havelock, Craven County.
Based on information from the provided plans, a sedimentation chamber and sand filter system is
proposed to meet the DENR high density stormwater requirements. As such, hand auger borings
were performed at 50 foot intervals 25 feet from the southerly property line. From general
observations of the existing and historical use of the property it appears the property has been
been vacant for some time. Per the Craven County soils map, the in -situ soils are of the Rain
series. Based on the observations and soil classifications, the generalized subsurface conditions,
the depth to seasonal highwater table appears to be approximately 2.8-3 feet.
FIELD EXPLORATION
Center of proposed sand filter
Topsoil to 6 inches, friable with coarse roots.
Beneath top soil was grayish -brown and sandy loam, with fine roots.
Groundwater table was encountered at 2.8 to 3 feet, elevation 45.2
Laboratory testing was not performed
