Setback Waiver:
In accordance with 15A NCAC 02T .0606(c), please submit the appropriate setback waivers (FORM:
NDWSW) that have been notarized, signed by all parties involved and recorded with the County Register
of Deeds. Waivers involving the compliance boundary shall be in accordance with 15A NCAC 02L .0107.
The setback waiver form provided with the application needs to be signed by Mr. Milstead and Ms.
Kempski, should be notarized and registered with the Durham County Register of Deeds.
