Section 401 Water Quality Certification for US 501 from NC 49 in Roxboro to
SR 1602 (Jesse Banks Road) in Person County, Division 5. Federal Aid Project No. STP-
501(11), WBS Element No. 34406.2.3, TIP No. R- 2241A.

Construction of TIP No. R -2241A is currently underway. At Station 76 +45 —L- RT (Plan Sheet 22,. Site
where the 750mm and 900mm RCP outlet to the base tail ditch, it was recently discovered that there
are issues with the elevations needed to provide positive drainage. The plan elevation of the base tail ditch
at the outlet of these pipes which have been installed is lower than the existing elevation of the stream bed
at the end of the TDE by roughly 8 -10 inches (0.25 meters). There is about a one foot drop in the
bed elevation approximately 46 feet down stream of the TDE. If the base ditch could be graded out to
this point then the ditch would drain and a ponding situation could be eliminated. Therefore the North
Carolina Department of Transportation (NCDOT) proposes to extend the base tail ditch another 46ft
(14m). Also for permanent stabilization rip rap armoring is proposed along the entire base tail ditch from
the outlet of the 750mm and 900mm RCP to the extent of the excavation (see attached revised permit
drawings). This proposal would result in the following change of impacts:
• decrease of 181 linear feet of temporary channel impacts (181 Ift changing from temp to perm)
• increase of 227 linear feet of permanent channel impacts (181 Ift that was temp + 46 Ift new perm)
o total permanent channel impacts at Site 18 now total 581 linear feet (181 Ift + 46 Ift + 354 Ift old)
Note�that since the stream impacts at this site are non - mitigatable, then the proposed increase of impacts
would not affect the mitigation for this project. Therefore no additional mitigation is proposed.

In coordination with the Project Development and Environmental Analysis Unit, application is hereby
made for a modification to the Department of Army Section 404 Individual Permit as required for the
above - described activities for the proposed TIP project R-224 IA. Also by copy of this letter we are also
hereby requesting a modification to the Individual Section 401 Water Quality Certification from the
