As discussed with you previously on the telephone, you can submit a written request to rescind the permit if the treatment system currently at this residence is not failing/discharging and you do not have plans to construct a new discharge system. The written request to rescind the permit must be mailed to the attention of Charles Weaver, Environmental Specialist, Compliance & Expedited Permitting Unit, Division of Water Resources, 1617 Mail Service Center, Raleigh, N.C. 27699-1617. The annual fee ($60.00 per year) has not been paid in the last several years. The invoices for these years have been included with this report. Please resolve these overdue payments as soon as possible. The on -site resident (renter) indicated that the septic tank is pumped one to two times per year with no observed failures of the subsurface disposal system.
