This letter is in response to .the
p question concerning condition 6 of
Baxter's land application permit number WQ0004222. Condition 6 prohibits
sludge application at times when the ground water level is 2 to 3 feet
beneath land surface.
Mr. Jack Floyd (Groundwater Section) stated that Permit WQ0004222's
condition 6 could be relaxed. The relaxation would require that Baxter
Healthcare Corporation apply for modification of their permit.
If only modification of the restricted months in condition 6 is
desired, please resubmit (in triplicate) updated copies of the
application form and supporting materials which were used to secure
Permit WQ0004222. A check in the amount of $400.00 must accompany the
updated applications. Baxter should cover the resubmitted permit
applications and documentation with a letter precisely describing changes
desired in condition 6. You may wish to also request dropping of the
currently permitted but unavailable fields in the cover letter.
If Baxter desires to add additional,_previously unpermitted sites to
Permit _W00004222, the attached copy of "Land Application of Residual
Solids" permit must be submitted in triplicate along with a $400.00 check
for the processing fee. A cover letter should accompany the application
forms precisely describing changes 'which .Baxter Healthcare Corporation
desires in the permit.

from Water Quality and studied is an application for an amendment to add 8 new
sites for application. Any groundwater quality monitoring, as deemed necessary
by the Division, shall be provided as in the existing permit.
