Please note that I will be handling this Civil Penalty Assessment appeal by PTR, Inc I
will need to begin preparing out case as soon as possible Please review the important dates
listed below and provide me with the requested information as soon as possible
October 27, 2000 Prehearing Statements and Document Constituting Agency
Action must be filed by this deadline with OAH I have requested
a copy of the civil penalty assessment document, but I also need a
copy of the assessment, the factors considered in assessing the civil
penalty, results of any samples taken, any correspondence
regarding the violation/ assessment, and any worksheets drafted
prior to the assessment Please send these materials as soon as
possible
Week Beginning January 22, 2001 Hearing is set in High Point, NC with
Sammie Chess, Jr, Administrative Law Judge presiding
