The Division of Water Quality has reviewed your submittal for a 404/401 Permit for after - the -fact
approval of stream culverting in Forsyth County Your application was not complete since no application was
received Your processing time will begin which we receive the public notice of the application from the U S Army
Corps of Engineers Please be aware that a 401 Water Quality Certification fee of $475 00 will be required for this
project before the 401 Certification can be acted upon This fee should be in the form of a check made out to the
"N C Division of Water Quality" Other likely requirements of the Division are on -site stormwater treatment
(likely a wet detention pond) and compensatory mitigation if the stream is perennial Please provide information
regarding the location of your project This information is needed by DWQ in order for us to decide whether this
project is approvable

request (by copy of this letter) that the Corps of Engineers place this project on hold Also, the project will be placed
on hold for our processing due to incomplete information (15A NCAC 2H 0507(a)(4)) Your processing time will
