Director Division of Water Quality - 4!_LE � - REG!L CFFJCE Enclosed is a report for the inspection performed on September 30, 2004 by Mr. Norm Good. Where deficiencies are cited in this report, a response is required as well as for all lettered comments and/or recommendations. Within thirty days of receipt, please supply this office with a written item for item description of how these deficiencies, comments and/or recommendations were corrected. If the deficiencies cited in the enclosed report are not corrected, enforcement actions will be recommended. For certification maintenance, your laboratory must continue to carry out the requirements set forth in 15A NCAC 2H .0800. We apologize for the delay is issuing this report. Mr. Good neglected to write the report for eight months. This is not our policy and, we strive to get reports out within 30 days of the inspection. INTRODUCTION: This laboratory was inspected to verify its compliance with the requirements of 15A NCAC 2H .0800 for the analysis of environmental samples. II. GENERAL COMMENTS: The laboratory is spacious and well equipped. All facilities and equipment are well maintained. Most records are well kept and most data looked good. Some further quality 'control procedures need to be implemented. At the time of the inspection our database did not include an email address for this laboratory. Electronic communications now frequently offer a more rapid and effective means of contact for all branches of state government An address was given (kradford7568@yahoo.com) at the time of the inspectionwhich the certification branch will use for contacting this facility. Please confirm that this is the correct address which your laboratory wishes to use for email communications with the certification branch. Of course most contact will continue to be made through the United States Postal Service. DEFICIENCIES, REQUIREMENTS, COMMENTS AND RECOMMENDATIONS: General A. Comment: The laboratory balance is not routinely verified with weights .throughout the range of use. Recommendation: Lab is checking the accuracy of the, balance with three weights daily. This practice satisfies the requirement of checking the analytical balance with at least one class S weight daily and at least three standard weights quarterly, Ref: 15A NCAC 2H .0805 (a) (7) (K). However, at the time of the inspection the three weights chosen were500 mg, 1000 mg and 2000 mg. One of these weights should be in the 100 mg and one in the area of 10 - 20 mg. Page 2 pH 1. Deficiency: Lab is generating results required by non -discharge permit WQ0003089 for pH compliance without certification for that parameter. Requirement: Municipallaboratories are required to obtain certificationfor parameters which will be reported to the State. to comply with State surface water monitoring, groundwater and pretreatment rules. Ref: 15A NCAC 2H .0804 (a) Suspended Solids 2. Deficiency: Lab had not, at the time of the inspection, calibrated the thermometer used in the suspended solids oven against an NIST traceable thermometer. Requirement: All thermometers must meet National Institute of Standards and Technology (NIST) specifications for accuracy or be checked, at a minimum annually, against an NIST traceable thermometer and proper corrections made. Ref: 15A NCAC 2H .0805 (a) (7) (0). IV. PAPER TRAIL INVESTIGATION: Paper trail investigation consisted of comparison of laboratory bench sheet and chain of custody documentation to discharge monitoring reports over the last three months. No errors in transcription or inconsistencies were observed. V. CONCLUSIONS: Correcting the above cited ., deficiencies and comments and implementing the recommendations will help this lab to produce quality data and meet certification requirements. DENR - FAYETTEVILLE REGIONAL OFFICE Your letter, received June 20, 2005 concerning corrective actions for your analytical procedures has been reviewed. You indicated by telephone on August 15, 2005 that Environment 1 had been contracted to perform the pH measurement and at this time you are withdrawing your request to add this parameter to the scope of your laboratory certification. The quality control measures taken in reference to the remaining deficiency and comment cited in the May 23, 2005 inspection report are acceptable. However, this report does not attempt to comprehensively address all certification requirements prescribed in the North Carolina Administrative Code (15A NCAC 2H .0800), promulgated methods, and applicable Code of Federal Regulations (40 CFR Part 136). It is the responsibility of the certified laboratory toaddress any required changes in Wastewater/Groundwater Laboratoty Certification application information, and to correct the Laboratoty Standard Operating Procedures and the Laboratory Quality Assurance Manual when requirements of the above cited codes are not in compliance. If a future inspection should reveal that the deficiency cited in the inspection report was not corrected, enforcement actions will be recommended in accordance with 15A NCAC 2H .0800.
