and evaluated. The results detailed below represent the status for each of your certified parameters and methods
to date. A blank in the "PE complete" column indicates no PE yet recorded for this calendar year. A "Pass" entry
indicates your lab has satisfied the annual performance evaluation requirement for that parameter and/or method.
Other information in this column indicates vendor and Study identification. A "Fail" entry requires a follow up
sample to be analyzed and reported to this office within 60 days. An N/A entry represents parameter exemption
from the performance evaluation requirement. Adate entered beside the "Fail" entry represents the completion
deadline for this PE follow up.

first miss. Failure to analyze and submit an acceptable follow up sample result within 60 days or producing a second
successive unacceptable result will result in decertification for that parameter.
