Dear Mr. Cooke:
After a review of your response to the October 7, 2021 Notice of Violation (NOV-2021-
PC-0324) from the Division of Water Resources (Division) Winston Salem Regional Office
and the Non -Discharge Permitting Branch, the Division requests that a permit
modification application be submitted within sixty (60) days of receipt of this letter. Per
North Carolina General Statute 143-215.1(a)(3) a permit is required to "alter, extend, or
change the construction or method of operation of any sewer system, treatment works,
or disposal system within the State."
The application should address the following modified system components-
1 . The piping connecting the mudwell in the package plant serving the reclaimed
system and the standard wastewater system.
2. The pipe in the package plant from the tertiary filters to the chlorine contact
chamber.
3. The pipe from the mudwell to the sludge basin.
4. The package plant tertiary filter media.
5. The modification to the pumps in the sand filter dosing tank of the standard
wastewater system.
An updated Operations and Maintenance Plan, engineer certification, and updated
system plans should be included with this permit modification. If you have any questions,
