Additional Information Requested: ® DWQ is unable accept this as a courtesy copy without referencing the Water Quality Certification number requested in Section I. Processing Item 2. Please state which Permit (Nationwide, Regional or General) number you intend to use. You may want to contact the U.S. Army Corps of Engineers (USAGE) to determine which permit they will allow you to use. ® An initial review of the application reflects 175 linear feet of stream impact.
