® RETURN OF CORRESPONDENCE
Dear Mr. Seyfarth:
Coleen H. Sullins, Director
Division of Water Quality
Polk County
07- 1 223
On July 2, 2007, the Division of Water Quality (DWQ) received correspondence dated June 26, 2007 for
the above referenced project.
Additional Information Requested:
® DWQ is unable accept this as a courtesy copy without referencing the Water Quality
Certification number requested in Section I. Processing Item 2. Please state which Permit
(Nationwide, Regional or General) number you intend to use. You may want to contact the
U.S. Army Corps of Engineers (USAGE) to determine which permit they will allow you to
use.
® An initial review of the application reflects 175 linear feet of stream impact. Please keep in
mind that dependent on how the Corps addresses this application, DWQ requires five (5)
complete application sets/maps along with the appropriate fee, which would be $475.00
Please call Mr. Ian McMillan or me at 919-733-1786 if you have any questions re~rding this matter.
