IV. ADDITIONAL REGIONAL STAFF REVIEW COMMENTS/ATTACHMENTS (Optional /IfNeeded)
The area of the proposed injection system was inspected on September 23, 2021. It is located along a street in
the former Cotton Mill Square (currently the Block 43 Cottages, a student housing area). The area of the
injection is flat (primarily a road and parking area) with easy access and located along the southeast boundary
of the property. There were no potential problems observed during the site visit. A pilot test conducted in
2019 indicated the proposed system was effective in remediating the contaminants of concern at the site.
View looking southwest of the proposed area of CAT 100 injection points. Former Extraction Well
EW-504 is visible to the right of the vehicle hood. The pilot test area was located to the right and
behind of EW-504. The injection points will be located along the right side of the road.

Reason for Visit: Routine
Inspection Summary:
Jim Gonsiewski of the WSRO on site at 10:10 AM 9/23/2021. Met with Greg Flasinski of Weston Solutions. Toured the
site. First visited the old remediation area. Previously a pump and treat system was located at the site. According to David
Kane of Weston Solutions, in 2009 the system was shut down due to extensive damage that occured during construction of
the Block 43 Apartments/Cottages student housing. 16 wells, a remediation system building, and associated underground
piping and valves were destroyed during the construction grading and work. Additionally, another 6 wells were damaged.
A remediation injection system (injection of diluted Newman Zone emulsified vegegtable oil in four injection wells across the
site) was permitted in February 2019. This system was abandoned in September 2019.
Observed the location of the new remediation system along Blunt street in the paved area along the southwestern boundary
of the site. Area appeared to have good access and room to install the new system.
