The North Carolina Division of Water Resources attempted to contact you for an
unannounced inspection of the septic treatment system at 495 Talley Road on September 30,
2021. Previous attempts to contact you via the phone number on file with your permit were
unsuccessful.
This routine inspection is meant to verify that the facility is operating in compliance with the
conditions and limitations specified in NPDES WW Permit No. NCG550715.
Furthermore, it appears that your permit is expired, and a renewal is due immediately.
Please see the enclosed renewal form for instructions. Prompt return of your renewal
paperwork is requested, as operating without a current permit is a violation of general
statutes and could result in enforcement action.
