Ken,
I just added up the proposed BUA for drainage area 2 on the revised sheet and it still doesn't match the total. In fact, it
still adds up to the same total amount as before (229,125 vs 214,750). If the total of 229,125 is correct, then I need you
to change that in both tables on the form.

V. SUPPLEMENT AND O&M FORMS
The applicable state stormwater management permit supplement and operation and maintenance (O&M) forms
must be submitted for each BMP specified for this project. The latest versions of the forms can be downloaded

VI. SUBMITTAL REQUIREMENTS
Only complete application packages will be accepted and reviewed by the Division of Energy, Mineral and
Land Resources (DEMLR). A complete package includes all of the items listed below. A detailed application
instruction sheet and BMP checklists are available from
http://portal.ticdenr.oriz/web/wq/tvs/su/statesiv/formes dots. The complete application package should be
submitted to the appropriate DEMLR Office. (The appropriate office may be found by locating project on the
interactiveoulmemapathttp://poital.ncdenr.org/web/wq/ws/su/ma ss.)
Please indicate that the following required information have been provided by initialing in the space provided
for each item. AB original documents MUST be signed and initialed in blue ink. Download the latest versions
for each submitted application package from http://portal.ticdeilr.org/web/nvcl/ws/su/statesw/forms_docs.
