Attn: Tom Wilson, Materials Manager 900 Industrial Drive Clinton, NC 28328 NORTH CAROLINA Environmental Quality October 5, 2021 Subject: COMPLIANCE EVALUATION INSPECTION NPDES Stormwater General Permit NCG030000 Dubose National Services, Inc., Certificate of Coverage NCG030703 Sampson County Dear Mr. Wilson: On September 29, 2021, a site inspection was conducted for the Dubose National Services, Inc. facility located at 900 Industrial Drive, Clinton, in Sampson County, North Carolina. A copy of the Compliance Inspection Report is enclosed for your review. Mr. Tim Shepard, Fabrication Division Manager and Ms. Tyler Miles, Environmental Compliance Managerwere also present during the inspection and their time and assistance is greatly appreciated. The site visit and file review revealed that the subject facility is covered by NPDES Stormwater General Permit NCG030000 under Certificate of Coverage NCG030703. Permit coverage authorizes the discharge of stormwater from the facility to receiving waters designated as Williams Old Mill Branch, class C, Sw waters in the Cape Fear River Basin. As a result of the inspection, the facility was found to be compliant with the conditions of the NCG030000 permit. Please refer to the enclosed Compliance Inspection Report for additional comments and observations made during the inspection. Please be advised that violations of the NPDES Stormwater General Permit are subject to a civil penalty assessment of up to $25,000 per day for each violation. The SWPPP contained the majority of the information required by General Permit NCG030000 except for the following information. There was no documentation of facility inspections which now need to be conducted quarterly according to the schedule in the General Permit.As a reminder, Analytical and Qualitative monitoring is also now required to be done quarterly. A list of significant spills or leaks during the previous 3 years or the notation that this has not occurred should be included in the SWPPP. The facility grounds were inspected including the outfalls and the best management practices. All appeared to be good condition with no issues noted.
