I am attaching a letter and related attachments that will update the Operator in Responsible
Charge (ORC) designations for the wastewater treatment plant to show Ronald as the ORC and
Fred as the back-up ORC. Both of the attached forms must be signed by both Ronald and Fred.
Once they are signed, please send the letter, along with the ORC forms and the copy of Ronald's
certificate to the State. I verified with. the State that Fred's request for a temporary Grade III
license has been approved. Since Ronald has now passed his Grade IV exam, he no longer needs
the Temporary Grade IV, so the State will be refunding the $200 check we sent with his request.

Dear Sir or Madam:
WestPoint Home (WPH) currently owns and operates a Grade IV wastewater treatment plant in
Wagram, NC under NPDES Permit No. NC0005762. The plant's current ORC is an independent
consultant — Johnny Hester. One of our employees, Ronald Locklear, is currently listed as the
back-up ORC. Ronald had been functioning in that position with a Temporary Grade III
Certification. Recently, Ronald has passed the tests to receive both his Grade III certification and
' his Grade IV certification (see attached certificate). I am attaching an Operator Designation Form
that now designates him as the Operator in Responsible Charge (ORC) of NPDES Permit No.
NC0005762.
Another of our operators, Fred Locklear, has also received his Grade II Certification (see
attached certificate) and has recently received a Temporary Grade III Certification so that he can
now serve as the back-up ORC. That is also shown on the attached form.
This facility also holds Land Application Permit No. WQ0008764, which was recently renewed
by the DWQ. One of the requirements of that new permit was to update the ORC information
within 30 days of that permit's effective date. Although this is slightly outside that window, we
have been holding off on the communication to determine whether Fred Locklear's Temporary
Grade III had been approved. That was confirmed last week. Attached is a second ORC form that
covers this permit.
