I would like to thank you and the Exchange Street facility staff for your time during the site visit. The Recycle system appeared well maintained and to be operating in accordance with the permit, at the time of inspection. I noted below the highlights of our discussions and records management needed to maintain overall compliance with permit requirements. • Submit the facility certification form attached to your permit (copy also provided during the inspection) in accordance with the instructions in permit condition 1.1. • Verify the drawings submitted with the application package are the "as built" drawings for the system; or, submit the final drawings as needed. • Hauling records of residuals removed from the facility need to be maintained in accordance with permit condition IV.3. • Maintenance records for the recycle system are currently logged in an off -site "main" computer data base tracking all facility repairs. It is acceptable to track this way, provided the recycle system information is readily available for review via an on -site computer. Reference permit condition IV.4. • The permit expiration date is 10/31/2018. Per permit condition 1.3., a renewal application is due 6 months prior to expiration. Please note: • The basin(s)/Recycle system appeared well maintained, at the time of inspection. This is a new system and records management was discussed as noted in the cover -letter. • The MRO should be informed of any changes to the contact information noted above. It appears as though freeboard will not be an issue. N Evidence of overflow Y Unusual color (very black, textile colors); Slurry is color of the concrete washed into the system. N Foam (are antifoam agents used) N Excessive solids buildup (from bottom); The tank bottom is not visible through the slurry; however, solids from the settling chambers are removed by the system, dried and stored until hauled. The first is after the "dirty" water settling chambers and the second at the filter press/aggregate reclaimer. Per conversation with Mr. De Lozier, the concrete is periodically hauled for reuse. A record keeping system for the hauling tickets and all system records was discussed, as noted in the cover -letter. Recordkeeping Y Is permit available upon request? N Are operational logs present?
