Division of Water Resources' Central and Regional staff has reviewed the application package
received June 22, 2021. However, additional information is required before the review may be completed.
Please address the items on the attached pages no later than the close of business on October 8, 2021.
Please be aware that you are responsible for meeting all requirements set forth in North Carolina
rules and regulations. Any oversights that occurred in the review of the subject application package are
still the Applicant's responsibility. In addition, any omissions made in responding to the outstanding items
listed herein, or failure to provide the additional information on or before the above requested date may
result in your application being returned as incomplete.
Please reference the subject application number when providing the requested information. All
revised and/or additional documentation shall be signed, sealed and dated (where needed), with one

1. The compliance and review boundaries should be delineated on all site maps or setback maps
(Figures 4, 5, 6 and 7). Please resubmit the maps which include a note indicating the delineation
of the compliance and review boundaries. e.g., "The compliance boundary is established at
(either 250 feet from the residuals land application area or 50 feet within the property
boundary, whichever is closest to the residuals land application area). The review boundary is
established midway between the compliance boundary and the perimeter of the residuals land
application area." [15A NCAC 02T .1104]
2. Also, according to the Map Guidelines (attached separately with the email), on all site maps
(Figures 4, 5, 6 and 7) please specify the following:
a. County where site is located
b. Landowner's name
c. Name of the Permittee and/or permit number
d. Site identification number or field number
3. In addition to what is stated in C.1 & C.2, on the site map for Field 01 (Figure 7), please specify
correct county where site is located, Perquimans in this case.
4. As per the site inspection conducted by the Washington Regional Office staff, a shooting range was
observed on southwest corner of Field 02-01, (`Site 2 Field 1' map submitted by Regional Office
staff has been attached for your reference). In addition to what is stated in C.1 & C.2, on the site
map for Field 02-01 (Figure 4), please show the location of the shooting range (considered as a
`place of assembly') while also delineating the applicable setback distances. Please also specify
whether the landowner or some other entity owns or manages the shooting range.
5. During the inspection, two public supply water wells were also observed and one of them is located
along Pender Road, at the northwest corner of Field 02-01. The other well is located at the northeast
corner. In addition to what is stated in C.1 & C.2, please revise the Figure 4 to show the locations
of the two public supply water wells while also delineating the applicable setback distance which
is 100 feet in this case. The 15A NCAC 02L rules prohibit water supply wells within the compliance
boundary. An alternative to this requirement can be approved in accordance with 15A NCAC 02T
.0105(h). Upon the Permittee's request, the compliance boundary may be located closer to the
waste disposal area due to an on -site water supply well. Maps showing wells with the re-established
compliance boundary must delineate or include a footnote describing the redefined compliance and
review boundaries.

As per our records, only one ORC, Mr. J. Bryan Smith has been designated as a backup ORC. To
designate an ORC, please submit a signed completed ORC designation form to the Water Pollution
Control System Operators Certification Commission for approval [15A NCAC 08G .0201].
