This inspection was conducted to verify that the facility is operating in compliance with the conditions and limitations specified in Ground Water Permit No. WI0700008. A summary of the findings and comments noted during the inspection are provided in the enclosed copy of the inspection report. The Compliance Evaluation inspection was conducted by Division of Water Resources staff from the Washington Regional Office. The following deficiency was noted during the inspection: Inspection Area Description of Deficiency System Operation Part II. 4. Sampling taps Compliance Issue: Part II. 4. Of Permit WI0700008 states, "The injection well system shall be constructed such that a sampling tap or other collection equipment approved by the Director provides a functional source of water when the system is operational. Such equipment shall provide the means to collect a water sample immediately after emerging from the water supply well and immediately prior to injection into the return well (15A NCAC 02C .0107 (j) (2). North Carolina 27889 252.94e.8481 Remedial actions should have already been taken to correct this problem and prevent further occurrences in the future. The Division of Water Resources may pursue enforcement action for this and any additional violations of State law. To prevent further action, carefully review these deficiencies and address the causes of non-compliance to prevent the recurrence of similar situations. Required Corrective Action: Sampling taps shall be installed as referenced above and in conformity with Permit No. WI0700008 to allow for collection of water samples. You are required to take any necessary action to address the above violation within 30 days of receipt of this letter. Notification shall be provided to the Division at the letterhead address within 30 days of receipt of this letter confirming completion of corrective actions. Please include in your response all corrective actions already taken and a schedule for completion of any corrective actions not addressed.
