As per our discussion, a copy of the completed inspection form has been enclosed to you. Part 5 of this report (Required Actions to be completed by Facility) contains Annotations 91-7 that Republic Services must complete. Please provide documentation that verifies Republic Services successfully completed the corrective measures to me by May 14, 2021. Please ensure a copy of this report is kept at the facility for records as well. The Stormwater Division reserves the right to enter Republic Services — Overdale Road Transfer Facility as often as necessary to determine compliance with the permit requirement of NC080917. The garbage is then loaded into semi -truck trailers to be taken to a landfill or incinerator. Some good condition roll -off containers are stored at the facility as well. Little to no trash was observed outside of the covered concrete slab where it was placed and being loaded. c. Stormwater staff was informed that the facility plans on installing silt sacks in the inlets but these were not installed yet. 2. Provide a brief description of other controls that manage/prevent/minimize Stormwater runoff (e.g., erosion and sediment controls, inlet protection/control at storm drains, diversion structures): Cleaning of the trash at the facility is done at the end of the work day. Oil -Water Separator catches all trash juice from storage and loading areas. Part 2 —Stormwater Discharge Outfalls (SDOs) 1. Any evidence of discharge to receiving waters/MS4 from the SDOs? (e.g.: stonnwater runoff, dry weather discharge, co -mingling of process wastewater, trash, debris, oil and grease): ®Yes ❑No Staining was observed at Outfall No. 2. Republic Services stated that the staining was being caused by asphalt run off from the parking lot and the road. (See Photo 1) 2) The spill kit must be replaced with a full kit for use on site in order to comply with permit requirements. (See Photo 1) 3) Scraps and trash located on the cast side of the processing building, must be removed from the facility to prevent potential pollutants from reaching the stormwater conveyances. (See Photo 2) 4) Outfall No. 2 needs to be evaluated for potential source of staining. The presence of the staining indicates pollutants reaching the stormwater outfall and the permit requires the permittee to determine a source of potential pollutants when they are observed. (See Photo 3) 5) Perform qualitative monitoring on a semiannual basis at both outfalls per the permit requirements. Please send copies of 1 years' worth of qualitative monitoring to Brandon Wise of the City of Winston-Salem Stormwater Division. 6) When pumping out the accumulated rain water from the secondary containment a drain log must be kept to document where water is pumped to and if any pollutant indicators were observed. 7) Keith Huff s information must be added to the spill response procedures in the S WPPP.
