On May 29, 2013, the Division of Water Quality (Division) received your application dated April
29, 2013, requesting a 401 Water Quality Certification and Buffer Authorization from the
Division for your project The Division does not currently have enough information to conduct a
thorough technical review of the two proposed wet detention ponds The application is on-
hold until the following information is received
1 Full -sized plan sheets showing the entire site with its storm drainage system, including
every inlet, outlet and conveyance device Please show how stormwater management
requirements have been met at every proposed outlet location
A BMP Supplement Form with all required items that have been omitted for each of the
two proposed wet detention ponds on the site (see
http / /portal ncdenr org /web /wq /ws /su /bmp- manual) Please be sure these items are
at the requested scales
3 An original signed and notarized O &M Agreement form Sample 0 &M forms are
available at http / /portal ncdenr org /web /wq /ws /su /bmp- manual
Pursuant to Title 15A NCAC 02B 0259 (8), the applicant shall furnish all of the above requested
information for the proper consideration of the application If all of the requested information

unable to approve the application and it will be returned The return of this project will
necessitate reapplication to the Division for approval, including a complete application package
and the appropriate fee
Please respond in writing within 30 calendar days of receipt of this letter by sending two copies
of all of the above requested Information to the Wetlands, Buffers, Stormwater — Compliance
and Permitting (Webscape) Unit, 1650 Mall Service Center, Raleigh, NC 27699 -1650
Please be aware that you have no authorization under the Water Quality Certification Rules for
this activity and any work done within waters of the state may be a violation of North Carolina
General Statutes and Administrative Code
