10, 2013, requesting a 401 Water Quality Certification and Buffer Authorization from the
Division for your project The Division has determined that your application is incomplete and
cannot be processed The application is on -hold until the following information is received
1 Please update the level spreader- vegetated filter strip design per Chapter 8 of the BMP
Manual, available at http / /portal ncdenr org /web /wq /ws /su /bmp- manual The DWQ
recommends that you consider using an engineered filter strip to reduce the length of
the level spreader needed and for ease of maintenance Please contact Annette Lucas
at (919) 807 -6381 if you have any questions about the design
Please provide the current version of the LS -VFS Supplement Form, including the
required items checklist and any items that were omitted from the previous submittal
The Supplement Form is available at the above web site
3 Please seal, sign and date all plan sheets
Pursuant to Title 15A NCAC 02H 0502(c) and Title 15A NCAC 02B 0233 (8), the applicant shall
furnish all of the above requested information for the proper consideration of the application
If all of the requested information is not received in writing within 30 calendar days of receipt

Please be aware that you have no authorization under the Water Quality Certification Rules for
this activity and any work done within waters of the state may be a violation of North Carolina
General Statutes and Administrative Code
