The North Carolina Department of Transportation ( NCDOT) proposes to replace a structurally deficient
bridge with a two barrel culvert on Irvin Farm Rd (SR 24 10) in Rockingham County. The project is located
east of Reidsville in Rockingham County. Replacement is necessary in order to reduce fixture maintenance
needs and increase traffic safety. This activity will result in permanent impacts to an UT to Troublesome
Creek. NCDOT is committed to an offsite detour. A sill will also be placed in one of the barrels to help
maintain low flow, stream width and stream integrity The NCDOT anticipates that the activity will be
authorized under Nationwide Permit (NWP) 14. This application is intended as a request for written
authorization from the US Army Corps of Engineers for the Nationwide 14. This package is also being
forwarded to the NC Division of Water Quality for authorization as prescribed under General Water Quality
Certification 03820. A DWQ buffer authorization is also requested. No road widening or additional travel
lanes are proposed. The following information is being provided to satisfy conditions of the Nationwide
Permit and General Water Quality Certification.

MINIMIZATION INFORMATION

Officer.
MITIGATION INFORMATION
Stream mitigation is anticipated from the US Army Corps of Engineers. Mitigation letter is enclosed.
Wetland mitigation is not anticipated, as wetland impacts do not exceed 0.10 acres and do not occur
in a unique natural wetland.

The bridge will be removed utilizing Best Mgt Practices and the site will be de- watered utilizing a pump around with silt
bags. Culverts will be installed at one foot depth. A concrete seal, two feet high, will be placed in the high flow pipe. The
low flow pipe will be aligned with thalweg of the existing stream. Conservation measures will include method II clearing
and design standards for sensitive watersheds. Equipment utilized will include backhoe and/or trackhoe.

Project Description:
NCDOT intends to replace Bridge No. 2 on SR 2410 (Irvin Farm Road) over an unnamed tributary of
Troublesome Creek. The proposed bridge replacement would be in the same location and at the same
alignment as the existing bridge. The proposed ROW is the same as the existing 60 -foot (roughly 18- meter)
ROW; minor ditch -line impacts are possible. An off -site detour is proposed. A study corridor measuring 300
feet (roughly 94 meters) from each end of the existing structure and 75 feet (nearly 23 meters) in each
direction from centerline, encompassing 2.183 acres (.884 hectares), was suggested. This area is considered
the APE for the purposes of this review.
SUMMARY OF CULTURAL RESOURCES REVIEW
Brief description o, f review activities, results o, f'rrview, and conclusions
On September 1, 2011 a review of the site maps and files archived at the North Carolina Office of State
Archaeology was conducted. No previously identified archaeological resources are recorded within the
project area. Additionally, consultation of the 1926 Rockingham County Soil Survey Map and the 1936
Rockingham County Highway Map revealed no previous structures within the project area (though the 1926
Soil Map does illustrate one structure northeast of the project area). No further archaeological investigations
are recommended for the project as currently described. If new ROW is acquired or if earth - disturbing
activities extend outside the above described APE, further review will be required.
Brief Explanation of why the available information provider a reliable basis for reasonably predicting that there an no
unrdenloed historiepmpertier in the APE:
As described above there is no existing evidence for archaeological resources in the project area. An
examination of LIDAR imagery of the project area suggests that the majority of the APE has been
landscaped for local residences or cleared and graded for highway or utility easements. Additionally, an
examination of the soil survey for Rockingham County suggests that much of the APE lees within frequently

SUMMARY OF CULTURAL RESOURCES REVIEW
Brief description of review activities, results of review, and conclusions:
Review of HPO quad maps, HPO GIS information, historic designations roster, and indexes was
undertaken on August 22, 2011. Based on this review, there are no existing NR, SL, LD, or DE
properties in the Area of Potential Effects, which is 75' from either side from the centerline of the bridge
and 300' from either end of the bridge. One SS property, RK1429, the John N. and Emily M. Irvin
House, is located approx. 2,700' west of the bridge outside of the APE, however the piece of land
associated with the parcel extends around the APE and occupies a portion of the eastern edge of the APE.
Google Maps "Street View" and HPO quad maps indicate that there are no structures associated with this
property within the APE, and ROW will not be acquired, this property will not be affected by this project.
There are several houses within the APE; Rockingham County GIS/Tax Information indicates that the
houses were built from 1967 and later, and all are less than fifty years of age. "Street View" confirms that
the houses are not NR eligible. No historic properties are present and no survey is required. In addition,
Bridge No. 2 is not NR eligible based on the NCDOT Historic Bridge Inventory.
Brief Explanation of why the available information provides a reliable basis for reasonably predicting
that there are no unidentified historic properties in the APE:
HPO quad maps and GIS information recording NR, SL, LD, DE, and SS properties for the Rockingham
County survey, Rockingham County GIS and tax information, and Google Maps "Street View" are
considered valid for the purposes of determining the likelihood of historic resources being present. There
are no historic resources present and no survey is required.

N.C. Department of Environment and Natural Resources' Ecosystem Enhancement Program In -Lieu Fee Instrument
dated July 28, 2010. If the above referenced impact amount% are revised, then this mitigation acceptance letter will
no longer be valid and a new mitigation acceptance letter will be required from EEP.
