Dear Mr. Hatchett:
On July 20, 2021 Division of Water Resources staff person Caitlin Caudle conducted a
follow up inspection of violations and deficiencies of items identified during the February
2021 compliance evaluation inspection. Glenn Price, ORC, and yourself were present
during the entire inspection.
A review of the treatment facility and some records was completed. The spray field
could not be reviewed at the time of inspection due to heavy rainfall making the access
road difficult to navigate. As a result, Ms. Caudle requested that you share photos of the
spray field and all additional information requested on March 22, 2021, by August 6,
2021.
The review of the facility and the additional information provided reflect the facility has
taken action to resolve the majority of the violations and concerns listed in the Notice of
Violation dated February 22, 2021. The following item has yet to be resolved:
Item A: Mr. Price provided an electronic copy of the Operation and Maintenance Plan to
Ms. Caudle. Please ensure that a copy of the Operation and Maintenance Plan is
on site per Permit Condition 111.1.

Reason for Visit: Follow-up
Inspection Summary:
On July 20, 2021 Division of Water Resources staff person Caitlin Caudle conducted a follow up inspection of violations and
deficiencies of items identified during the February 2021 compliance evaluation inspection. Glenn Price, ORC, and Jerry
Hatchett, Maintenance Director, were present during the entire inspection. A review of the treatment facility and some
records was completed. The spray field could not be reviewed at the time of inspection due to heavy rainfall making the
access road difficult to navigate. Mr. Hatchett is going to provide Ms. Caudle with photos of the repairs and improvements
completed in the spray field.
Mr. Hatchett stated he is gathering items to provide for the additional information request sent by Ms. Caudle on March 22,
2021. Mr. Price and Mr. Hatchett are working to designate a new back up ORC. Mr. Price stated he calibrated the drip
system in June of 2021 and will provide the records via email. The septic tank listed in the permit description is still used
and residuals are pumped yearly. Mr. Hatchett is including records of residuals removal with the add info response.
Mr. Price stated that the O&M plan was not on site, as referenced in Item A.
Mr. Hatchett stated that the following items have been completed:
Item B: Saplings have been removed from the spray field and limbs trimmed from the spray pattern. The trees downed from
the last storm have been removed. All sprinkler heads have also been replaced and the isolating valves on the risers
checked.
Item C: Two new combination locks have been placed on the gate to the spray field, and the fence has been repaired.
Item D: The lagoon embankment has been mowed and the saplings on the embankments cut down. A small patch of
vegetation was still present on the lagoon liner. Mr. Hatchett stated that he will monitor the bare areas on the toe slope of
the lagoon embankment as it is hard to establish vegetation.
Item E: Mr. Hatchett stated that the irrigation pump has been repaired, the soil was not suitable for irrigation so this could
not be confirmed. A whole was found in a pump house wall that faced the lagoon embankment This hole was close to rip
rap and could allow stormwater to enter the pump house. Please repair the hole as soon as possible.
Mr. Hatchett stated that he has been working on the permit renewal application and should be able to send it by the end of
the week.
Other observations:
• Mr. Hatchett asked about the life span of lagoon liners as this liner is 21 years old.
• Mr. Hatchett state that the stream crossing for the supply line pipe is above ground and he checks the crossing about 3
times a year. Ms. Caudle requested that he provide a map that marks the approximate location of the stream crossing.
• There is a grease trap at the facility that is inspected periodically, but the friers have been removed from the facility. The
majority of flow through the grease trap is just sink water.
• The plastic chlorine contact chamber has been removed from the inlet pipe and a single tablet storage pipe placed in
front of the effluent inlet pipe. Mr. price stated that this was because of the low flows which did not allow effluent to flow
through the chlorine contact chamber. Tablets were not visible in this singular tube. Currently there are only 103 students at
the elementary school.
• The concrete chamber containing the chlorine contact chamber was full of duckweed and other solids. There appeared
to be a sludge mat developing in the bottom of the chamber. This may need to be checked during the next compliance
inspection.
A permit renewal application was received on August 2, 2021.
The additional information requested was received on August 2, 2021, including photos of the irrigation field.
