Box 67 Jefferson, NC 28640 SUBJECT: Permit No. NC0021709 Authorization to Construct Town of Jefferson Wastewater Treatment Facilities EPA Project No. C370710-01 Ashe County Dear Mr. Dixon: The final plans and specifications for the subject project have been reviewed and found to be satisfactory. Authorization is hereby granted for the construction of modifications to expand the existing 0.15 M® wastewater treatment plant to a 0.30 MGD facility to consist of modifications to the existing three influent pump impellers to obtain two (2) 325 gpm pumps and one (1) 225 yiatt pump, an ultra sonic flow meter, a flow splitter box, 51,836 gallons of additional aeration capacity, upgrading the existing blower motors to dual 30 hp blower motors, a new 34 foot diameter secondary clarifier, a return/waste sludge pump station with dual 200 you submersible pumps, waste sludge and return sludge flow meters, a 65,650 gallon aerobic digester with dual 400 cfm multi -stage centrifugal blowers, modifications to the existing sludge digester and secondary clarifier to provide 11 days of sludge storage, ultra -violet disinfection, a v-notch weir with recording flow meter, associated yard piping, electrical work, sitework and appurtenances. The permittee shall employ an actively State certified wastewater treatment plant operator to be in responsible charge of the wastewater treatment facilities. Such operator must hold a valid State certificate of the grade at least equivalent to the classification assigned to the wastewater treatment facilities by the Certification Commission. When a new chief operator must be obtained to be in responsible charge of the new facilities, it is required that this be done by the time the construction of the facilities is 50% completed. This Authorization -to -Construct is issued in accordance with Part III, Paragraph C of NPDES Permit No. NC0021709 issued June 10, 1987 and shall be subject to revocation unless the wastewater treatment facilities are constructed in accordance with the approved plans, specifications, and supporting documents; and are in cunuliance with the conditions and limitations specified in Permit No. NC0021709. The permittee shall have an acceptable sludge analysis and a sludge E.P. toxicity analysis which have been approved by the Division of Health Services, Solid Waste Management Branch prior to the disposal of any sludge from the new wastewater treatment plant. Copies of the analyses, supporting documents, and approvals shall be submitted to the Division of Environmental Management's Water Quality Section and the Raleigh Regional Office. During the construction of the proposed additions/modifications, the permittee shall continue to properly maintain and operate the existing wastewater treatment facilities at all times, and in such a manner, as necessary to comply with the effluent limits specified in the NPDES Permit. The connection of the existing force main to the new splitter box must be done between the hours of 12 AM and 5 PM. You are reminded that it is mandatory for the project to be constructed in accordance with the North Carolina Sedimentation Pollution Control Act and, when applicable, the North Carolina Dam Safety Act. In addition, the specifications must clearly state what the contractors responsibilities shall be in comply with these acts. The selling or free distribution of wet, dried, composted sludge or sludge -like materials from wastewater facilities without prior review and approval by this Division is illegal. • One (1) set of final approved plans and specifications is to be forwarded to you, one (1) set to the Environmental Protection Agency (EPA), and one (1) set to the Corps of Engineers.
