You only need to upload the final approval of the 0.88 acres then and just explain that it covers modified disturbed acres
for the original lots + new disturbed area for new lots 1, 6, 7, & 9. 1 would just do this with one Modification submittal.

The tracking ID number is not really something we use. It was incorporated into the original form early
on, but we really use the COC number is the primary identifier. All that permit info on the Mod form
will be filled out automatically when you enter the COC number.
The email approval from Winston-Salem is what we would expect to see for the additional disturbance
approval. The .88 is the project total cumulative disturbed acreage, which includes lots 5 & 19 from our
original COC NCC211661 as well as lots 1, 6, 7 & 9. Does that include the modified original disturbed
area as well, or was there a different communication about that? I take it the 0.88 acres is for lots 6, 7,
1, 9 that you indicated on the mock-up form? Yes .88 is total cumulative disturbed acreage including 1,
5,6,7,9&19.
You can submit the revision in original disturbed area as a separate modification or as part of this one;
that's completely up to you. Just be sure to specify in the notes what it covers. I would need to say it
covers 1, 5, 6, 7, 9 & 19, correct?
It just seemed like the attachments you sent were only for the additional lots. I have the "review
completed" email when we submitted lots 6 & 7 showing total disturbed as .54 as attached - do you
need this uploaded as well? Are there any other documents I need to upload?

Thank you Bethany! I certainly appreciate your help with this - it sounds like I am able to do the
modification of disturbed acreage from the NCC213436 as well as submit the new disturbed acreage at
the same time (referenced in previous email).

Thank you for submitting your project to City of Winston-Salem. Review of
the plans for this project have been completed. The design is adequate to
meet the ordinance standards and the plans have been approved.
If you have not already done so, please provide an original signed and
notarized Financial Responsibility/Ownership form. This document is
required prior to the issuance of the Grading/Erosion Control Permit. If this
has already been provided, then please disregard this statement.
If this project will disturb more than 1 acre or is part of a larger common
plan of development, the Grading/Erosion Control Permit cannot be issued
until documentation is provided that coverage under the NCG01
Construction Stormwater Permit (NPDES) has been obtained. Please
obtain coverage under the permit at the following website
(-1tzP5:L i r-UUL5-UU nL.yuv/ tormb NCG01-N0' ). Once coverage is
obtained please upload a copy to the project documents in the electronic
plan review system. The Grading/Erosion Control Permit will not be issued
until proof of coverage under NCG01 is provided.

In order to hopefully get this right the first time, do you mind reviewing the attached to verify the
correct info will be entered to submit the modification for NCC213436 and additional disturbed acreage
I have just gotten approved for the new one? I am also attaching the supporting documentation to
verify that is what you are looking for.

what is understood, there would not be additional $100 fees required as we add more

I'm not sure I understand what needs to be "voided"? This sounds more like you need
to submit a Modification for this COC no. NCC213436 to revise the disturbed area
approval. You can do that here:

There is no fee for a modification. None of the information is changed on the original
COC, but you can access the modification supporting documents anytime in our
Laserfiche repository for this permit.
In addition, I do need to submit another NCG01 app for this same project, in which I
have the link to get started - how do I attach it to the original?
If it's for the same project, you can just add disturbance approvals and other
documentation with the Modification form. If you are looking to obtain a separate COC
for some reason, then a new eNOI is appropriate. There is not really a way to "attach"
a new one to the original COC, but if I know which one it is, I can create short-cuts
between the permit folders so they are linked / accessible easily from inside each set
of documents.

I hope you are well. Per your voicemail greeting requesting corresponding via email,
this a two part email with several detailed questions, for which I hope you can assist or
provide me with the correct contact please. This is in regard to "Briarcliffe Village SFD
Construction" in Forsyth County / Winston Salem- The Veritas Group, Inc. as permittee.
1. 1 am following up on our latest NCG01 submission (dated 6/3/21 affiliated with
project ID 500853 / NC Reference No. NCG01-2021-3436 / Certificate of Coverage
No. NCC213436 as attached). If you need anything further, I am happy to provide any
other info I have.

2. In addition, I do need to submit another NCG01 app for this same project, in which I
have the link to get started - how do I attach it to the original?
Would you confirm the following is how I need to go about the application:
In 1a, Briarcliffe Village SFD Construction (in the past I have supplied the same project
name)
In A3, I would use the original address this project was issued by our local authority as
2707 Zuni Ct
A7 with original [at 35.991880
A8 and original long as-80.289040
Al but use the cumulative total site acreage as 1.41
Al and cumulative total acreage to be disturbed as .88.
A14 and include cumulative total post -construction impervious area (I don't have that
cumulative figure)
Would I then in: Part A: 1b, 1c and all of Part D - use property specific info for my new
submission as well as the NO1 Cert form?
Also, the link we received to submit the new application referenced above did not
come with a project tracking ID - the link is a generic link. Is this correct and should I
anticipate this change going forward?
I certainly appreciate your assistance and time, especially given due to clarity needed
of these details.
