Inspection Summary:
This inspection was in response to a complaint of foaming water in a downstream pond. A new employee dumped cleaning
agents into a storm drain. The facility has monitored the discharge and the last rain event flushed the remaining soap
through the system. The Permittee has retrained all'of the staff responsible for maintenance and is updating their SWPPP
to include storm drain locations and pollution prevention training at all initial trainings for new employees. However, based on
this illicit discharge and evidence of past releases of pellets, Fiber Composites LLC must submit to this office by September
10, 2020 an action plan with deadlines to 1)implement all reasonable measures, available to minimize resin pellet releases
and any other process materials to any storm drain, 2) evaluate measures to diverty as much stormwater as possible away
from Mr. Rhodes pond and 3) an action plan to remediate Mr. Rhodes pond. Failure to meet the September 10,2020
deadline will result in a Notice of Violation and enforcement action. If you have questions, contact James Moore at
