On July 21, 2021, I conducted a startup inspection of the two new storage structures (i.e., MATHIS-1 Storage Facility).
We have received the PE Certification for both structures and they appear to be installed as proposed. A waste level
gauge is installed in each structure. This satisfies Permit Conditions 1.1., 1.2. and 1.3. You may start using the storage
structures when you are ready.
As a reminder, please be sure to conduct the inspections and record keeping for these structures as required in Permit
No. WQ0038087.

Inspection Summary:
On July 21, 2021, WSRO staff conducted a site visit to the new storage structures (i.e., source MATHIS-1). The purpose of
the site visit was for a startup inspection of the structures. Mr. Luke Mathis, ORC accompanied staff on the visit. The
storage structures and associated items appear to be installed as proposed and be ready for use. The locking valve for the
secondary containment was locked at the time of the visit. Onsite representatives indicated that David Coyle and the primary
ORC are the only two that have keys to open this locking valve to release stormwater as needed. The engineer certification
was received on July 16, 2021 along with an as built site map. Permittee was notified that startup inspection completed and
new storage structures can be used when they are ready to start.
