This is in response to your submissions dated September 29, 2000 and March 30, 2001 which
were received on October 5, 2000 and April 2, 2001, respectively, of plans and specifications
for the repair of the subject dam in Moore County on Mill Creek in compliance with the Dam
Safety Law of 1967. These plans and specifications were prepared under your supervision.
This letter constitutes approval of the proposal to repair the subject dam according to the plans
and specifications submitted to this Division on March 30, 2001 with the following stipulations:
1. A minimum flow of 0.5 cubic feet per second must be released from the dam site at all
times, even during construction.
2. Project construction shall be performed under your supervision.
3. During construction the Division of Land Resources may require such progress reports
as are deemed necessary.
4. Prior to the application of the specified joint sealant, the manufacturer's recommended
primer shall be applied to the concrete.

5. Records of maximum grouting pressure and volume of grout take shall be provided with
the "as -built" drawings.
6. Upon completion of the project, you shall inspect the completed work and upon finding
(a) that the work has been done as specified, and (b) that the dam is safe, and (c) that
minimum stream flow requirements have been satisfied, shall file with the Division of
Land Resources two sets of "as -built" drawings and a certificate stating that the work
has been completed in accordance with approved plans, specifications and other
requirements.
7. The lake level shall be lowered and maintained at the lowered lake level until final
approval is issued by the Division of Land Resources for operation of this dam. Also,
draining of the lake shall be performed in such a manner as to preclude off -site
sedimentation.
8. You must notify Mr. William E. Vinson, Jr., P.E., Regional Engineer, Land Quality
Section, 225 Green Street, Suite 714, Fayetteville, North Carolina 28301, (910) 486-
1541 ten days before the start of construction.
