Three sets of detailed plans and specifications signed and sealed by a North Carolina Professional Engineer. The plans must include a general location map, a plan view of the sewer extension, a profile of the sewer extension, and must show the proximity of the sewer extension to other utilities and natural features. Specifications may be omitted for delegated authorities. Each sheet of the plans and the first page of the specifications must be signed and sealed. The letter should be a recent letter and should refer to the project by the same name as that identified on the application and the plans/specifications. Tedder, Chief Water Quality Section Subject: Delegated Municipalities Checklist for Revised Non -Discharge Permit Applications The Division of Environmental Management's Water Quality Section has revised the Gravity Sewer Non -Discharge Permit Application Form. The form now incorporates the basic information that the Division feels is necessary in order for the delegated municipalities to perform a review on a gravity sewer project for the Division; therefore, the Division will no longer require that the "Delegated Municipalities Gravity Sewer Design Checklist" be completed and submitted. We do, however, require that the delegated municipality perform the same type of review as always required and complete the bottom portion of this page and attach it as the last page of all gravity sewer applications that are being submitted as a delegated municipalities project. The following items must be addressed before we can complete our review: 1) The Division requires that at all stream crossings, either the top of the pipe be below the stream bed (at least three feet unless ferrous material pipe is used) or that the bottom of the pipe be above the 25 year flood elevation. Please provide the 25 year flood elevation on sheet 6 (between station 6+56 and station 6+95 and between station 8+77 and station 9+55) of your plans to confirm compliance with this requirement. If 25 year flood information is not available, or if the aerial crossing is not above this elevation, please provide written confirmation that no damage will be incurred by upstream or adjacent properties due to this aerial crossing, and that the crossing will be properly inspected and maintained. If necessary, please provide 3 signed and sealed copies of the revised plans to comply with the above requirements. Refer to the subject permit application number when providing the requested information. Please submit three (3) copies of all information to my attention at the address below.
