In our permit application, we had specified 1.5 inch diameter PVC injection wells; for reasons of cost and capacity, we are considering installing 2-inch diameter wells. Dear Mr. Wood: In accordance with the application received on September 24, 2010, and the supporting data received December 17, 2010, we are forwarding permit number WI0600064. This permit is to inject Anaerobic BioChem to remediate groundwater contaminated with chlorinated solvents at the facility referenced above. This permit shall be effective from the date of issuance until November 30, 2013, and shall be subject to the conditions and limitations stated therein, including the requirement to submit a final project evaluation as stated in PART VII — MONITORING AND REPORTING REQUIREMENTS. Please note that Part VII.2 of the permit requires the submission of an updated site map with as -built injection well locations. Please read the entire permit to ensure that you are aware of all compliance requirements of the permit. You will need to notify this office by telephone 48 hours prior to initiation of operation of the facility. In order to continue uninterrupted legal use of the injection facility for the stated purpose, you must submit an application to renew the permit 120 days prior to its expiration date. In item IV(B)3 of the application form, the injection volume range stated (550-650 gallons) is only for the ABC substrate; in hindsight I see that the total volume of fluid should have been entered. In Attachment D, we describe our proposed total injection volume of 570 gallons of ABC (within the range identified above) mixed with 2,130 gallons of potable water, for a total volume of 2,700 gallons. Therefore, the correctentry for item IV(B)3 should be a volume range of 2600 to 3100 gallons total fluid (ABC and water) injected per event. I am pleased to learn that our application is nearly through the review procedure, and we are eager to move forward with groundwater remediation at this site.
