INFORMATION IN SUPPORT OF APPLICATION
FOR RENEWAL OF PERMIT WI0700012
FOR OPERATION OF INJECTION WELL SYSTEM
ITEM 1: INJECTION PROCEDURE
The tank farm area remedial action system injection wells have been in operation
since March 24, 1992. The injection well system contains seven 4-inch diameter
PVC wells generally 49 feet deep tapping the 17 to 22-foot thick Croatan Formation
with its top located 27 to 31 feet below ground surface. These seven wells are
designated CIW-1 through CIW-7 (Croatan Injection Well). The seven wells are
connected by a common 4-inch diameter PVC header pipe at Elevation 4.0 feet
(MSL) connected to a water level control tank near injection well CIW-4. The
injection well system is designed to operate at a water level between 5 and 15 feet
(MSL) and at a flow rate up to 40 gal/min. Approximately 33% of the injection water
is captured by the Croatan Formation withdrawal wells and 67% flows radially east,
west and south away from the injection wells. The construction plans and materials
and operation procedures were described in the original permit application submitted
on March 7, 1991, and are on file with the Department.
The injection wells are scheduled to continue to operate 24 hours/day, 365
days/year. The injection of fresh water will continue until the target clean-up values
established for the remedial action are achieved at the withdrawal wells. For the last
monitoring period for which data have been reported (i.e., May 2007, June 2007,
and July 2007), the injection rate averaged 27.9 gal/min. Similar injection rates are
projected in the future.
ITEM 2: HYDROGEOLOGIC DESCRIPTION
The tank farm area remedial action system was documented in the Ardaman &
Associates, Inc. Remedial Action System Design report dated July 31, 1990 and
approved by the State of North Carolina Department of Environment, Health and
Natural Resources Division of Environmental Management — Groundwater Section
in their letter of November 26, 1990. Section 2 of this document should be referred
to for details regarding hydrogeologic conditions in the tank farm area.
ITEM 3: MONITORING PROCEDURE
Monitoring of the injection well system will continue in accordance with the
performance monitoring plan already in place as part of the remedial action plan,
and the Part V requirements of the existing injection well permit, WI0700012.

Quarterly reports will continue to be submitted to the Department in the same format
as currently used. (Attachment VII)

ITEM 4: SOURCE WELL INFORMATION
Water for the injection well system consists of water from the depressurizing well
water system on the western boundary of the current mine site. The depressurizing
wells tap the upper unit of the Castle Hayne limestone with open hole collection
zones within the formation typically extending from 180 to 280 feet below land
surface. The upper 180 feet of the wells, through the undifferentiated surficial
deposits, Croatan Formation, Yorktown Formation, Pungo River Formation and
upper,20-to 30 feet of the Castle Hayne limestone, are sealed with a grouted steel
pipe casing.
The depressurization well water is pumped to the chemical plant complex through a
piping system.
ITEM 5: OTHER WELL DATA
The locations of the seven withdrawal wells (CWW-1 through CWW-7) tapping the
Croatan Formation (i.e., the injection zone) are shown on the attached plan and
described in detail in the Remedial Action System Design report on file with the %
Department. Well logs for the seven withdrawal wells are included in Attachment It
The locations of seven wells and piezometers tapping the Croatan Formation used
to monitor the performance of the injection well system (MWTF-1C, MWTF-2C,
MWTF-3C, CPZ-1, CPZ-2, CPZ-3 and CPW) are shown on the attached plan, and
the well logs are included -in -Attachment -IV.
ITEM 6: INJECTION —RELATED EQUIPMENT
The layout of the tank farm area remedial action system including the Croatan
Formation injection well system was described in the Remedial Action System
Design report and the original Permit application submitted on March 7, 1991, both
of which are on file with the Department. The only significant changes to the system
have been: (i) to substitute a multimedia anthracite/sand filter and an ultraviolet tube
light filter for the reverse osmosis unit described in the original permit application;
and (ii) substituting depressurizing well water for water from the FERT-1 well
described in the original permit application.
ITEM 7: WELL LOCATIONS
A plan showing the locations of the injection wells was presented in the tank farm
area Remedial Action System. Design report. A copy of that report is on file with the
Department. The attached figure also depicts the locations of the seven injection
wells, seven withdrawal wells and seven wells/piezometers used for performance
monitoring. The locations of other remedial action system wells in the tank farm
area are also shown on the figure.

ITEM 8: INJECTION FLUID DATA
The injection fluid consists of groundwater from the upper unit of the Castle Hayne
limestone. Wells providing the injection fluid have collection zones tapping the
Castle Hayne aquifer between depths of about 180 to 280 feet below land surface.
Prior to injection, the groundwater is treated by a multimedia anthracite/sand filter
and ultraviolet light filter. The quality of the injection fluid is monitored quarterly and
reported to the Department. The last set of analyses performed on the injection fluid
on April 25, 2007 are tabulated below and compared with the current permit
concentration limits.

ITEM 9: PERMITS
The original permit to construct and operate the injection well system was submitted
to the North Carolina Environmental Management Commission Department of
Natural Resources and Community Development (Form GW-57B) on March 7, 1991.
The permit, 06-0071-40-0005, was issued by the Department on December 5, 1991.
The permit was amended on June 15, 1995 (with the new company name and
permit number). An application to renew the original permit was filed on May 8,
1996, and the permit was renewed by the Department of September 23, 1997 as
Permit No. WI0700012. An application to renew WI0700012 was submitted on
March 30, 2001, and the permit was renewed by the Department on March 19, 2003.
The expiration date for the permit is January 15, 2008.

PCS PHOSPHATE - TANK FARM REMEDIAL ACTION PLAN
QUARTERLY OPERATIONAL AND MAINTENANCE REPORT FOR
THE INJECTION AND WITHDRAWAL WATER WELLS
May 2007 — July 2007
The Tank Farm Remediation System consists of a series of seven- (7) injection water wells located on the
south side of the Tank Farm Area and a series of fourteen (14) withdrawal water wells located on the north
side of the Tank Farm Area.
Injection Water Well System
The injection water well system operated with the DPW Water - Ultraviolet Light Disinfection system
during the period.
Daily operational data are collected by the Shipping Department and reported to the Environmental Affairs
Department. The total monthly and quarterly volumes of water injected as well as the month end water
level in the Water Level Control Tank are:

Withdrawal Well System
The withdrawal water well pumps are monitored daily by the area Maintenance Department and
preventative maintenance work is performed as needed. The total monthly and quarterly withdrawal water

Some additional short periods of planned downtime for preventative maintenance, pipe header cleaning,
and troubleshooting on individual pumps in the fourteen well system were experienced. Details of these
activities are:

All Pumps were flushed on 5/1, 5/11, 5/25, 5/30, 6/29, 7/10, 7/18 and 7/26.

In accordance with our phone conversations on March 13 and March 18, 2003, we are. amending
Permit number W10700012. This permit is for the injection of potable water to control the
migration of contaminated groundwater at the Tank Farm site at PCS Phosphate's plant in
Aurora, NC. This amended permit corrects a mistake in the monitoring schedule that was
included in the permit issued on December 19, 2002. Specifically, the monitoring schedule in
Part V, Section B,.paragraph 1, has been changed to require sampling for several parameters
from CIW7 and MWTF-ICR in April and October, rather than January and October. Please note
that the quarterly sampling schedule for total coliform organisms is unchanged.
This permit replaces the permit issued on December 19, 2002. The permit shall be effective
from the date of issuance until January 15, 2008, and shall be subject to the conditions and
limitations stated therein. Please read the entire permit to ensure that you are aware of all
compliance requirements of the permit. In order to continue uninterrupted legal use of the
injection facility for the stated purpose, you should submit an application to renew the permit
three months prior to its expiration date.

PERMIT FOR THE CONSTRUCTION AND OPERATION OF A WELL FOR INJECTION
In accordance with the provisions of Article 7, Chapter 87; Article 21, Chapter 143, and
other applicable. haws, Rules, and Regulations
PERMISSION IS HEREBY GRANTED TO
PCS Phosphate Company, Inc.
FOR THE OPERATION OF A TYPE 5Z INJECTION WELL SYSTEM, defined in Title 15A
North Carolina Administrative Code 2C .0209(e)(3)(C), consisting of 7 injection wells - for the
purpose of injecting water to control migration of contaminated groundwater. This well system
is located in the Tank Farm area at PCS Phosphate Company Inc., Aurora, North Carolina, in
Beaufort County, and will be operated in accordance with the application dated April 3, 2001 and
in conformity with the specifications and supporting data submitted October 7, 2002, all of
which are filed with the Department of Environment and Natural Resources and are considered a
part of this permit.
This permit isfor Operation only, and does not waive any provisions of the Water UseMt or
any other applicable Laws, Rules, or Regulations. Operation and use of an injection well shall be
in compliance with Title 15A North Carolina Administrative Code 2C .0100 and .0200, and any
other Laws, Rules, and Regulations pertaining to well construction and use.
This permit shall be effective, unless revoked, from the date of its issuance until January 15,
2008, and shall be subject to the specified conditions and limitations set forth in Parts I through
X hereof.

PART I = GENERAL CONDITIONS
2. The Permittee must comply with ail conditions of this permit and with the standards and
• criteria specified in Criteria and Standards Applicable to Injection Wells (15A NCAC 2C •
.0200). Any noncompliance with. conditions of this permit constitutes a violation of the
North Carolina Well Construction Act and is.grounds for enforcement action as provided for
in N.C.G.S. 87-94.
3. Each injection well shall be secured to reasonably insure against unauthorized access and use.
Each well shall be permanently labeled with a warning that it is for injection purposes and the
entrance to each well must be secured with a locking cap.
4. Each injection well shall be afforded reasonable protection against damage during
construction and use.
5. Each injection well shall have.permanently affixed an identification plate..
6. This permit is effective only with respect to the nature, volume of materials and rate of
injection, as described in the application and other supporting data: •
7. This permit is not transferable without prior notice to, and approval by, the Director of the
Division of Water Quality (Director). In the event there is a desire for the facility to change
ownership, or there is a name change of the Permittee, a formal permit amendment request
must be submitted Co the Director, including any supporting materials as may be appropriate,
at least 30 days prior to the date of the change.
8. The issuance of this permit shall not relieve the Permittee of the responsibility of complying
with any and all statutes, rules, regulations, or ordinances which may be imposed by other
local, state, and federal agencies which have jurisdiction. Furthermore, the issuance of this
permit does not imply that all regulatory requirements have been met.
PART II - PERFORMANCE STANDARDS ,
1. The injection facility shall be effectivelymaintained and operated at all times so that there is
no contamination of groundwater which will render it unsatisfactory for normal use: In the:
event that the facility fails to perform satisfactorily, including the creation of nuisance
conditions or failure of the injection zone to adequately assimilate the injected•fluid, the
Permittee shall take immediate corrective actions including those actions that may be
required by the Division of Water Quality such as the repair, modification, or abandonment
of the injection facility.
2.. The Permittee shall be required to comply with the terms and conditions of this permit even
if compliance requires a reduction or elimination of the permitted activity.
3. The issuance of this permit shall not relieve the Permittee of the responsibility for damages to
surface or groundwater resulting from the operation of this facility.

PART III - OPERATION AND MAINTENANCE REQUIREMENTS
1. The injection facility shall be properly maintained -and ;operated at all times.
2. The Pennittee must notify the Division and receive prior written approval from the
Director of any planned physical alterations or additions in the permitted facility or activity
not specifically authorized by the permit.
PART IV -INSPECTIONS
1. Any duly authorized officer, employee, or representative of the .Division of Water Quality
may, upon presentation of credentials, enter and inspect any property, premises, or place on
or related to the injection facility at any reasonable time for the purpose of determining
compliance with this permit, may inspect or copy any records that must be maintained under
the terms and conditions of this pemut, and may obtain samples of groundwater, surface
water, or injection fluids.
2. Department representatives shall have reasonable access for purposes of inspection,
observation, and sampling associated with injection and any related facilities as provided for
in N.C.G.S. 87-90.
3. Provisions shall be made for collecting any necessary and appropriate samples associated
with the injection facility activities.
PART V - MONITORING AND REPORTING REQUIREMENTS
SECTION A
1. Any monitoring. (including groundwater, surface water, or soil sampling) deemed necessary by
the Division of Water Quality to insure surface and ground waterprotection, will be established
and an acceptable sampling reporting schedule shall be followed.
2. The Pennittee shall report by telephone, within 48 hours of the occurrence or first knowledge
of the occurrence, to the Washington Regional Office, telephone number [phone], any
of the following:''
(A)Any occurrence at the injection. facility which results in any unusual operating
circumstances;
(B) Any failure due to known or unknown reasons, that renders the facility incapable of
proper injection operations, such as mechanical or electrical failures.
3. Where the Pennittee becomes aware of an omission of any relevant facts in a permit
application, or of any incorrect information submitted in said application or in any report to

the Director, the relevant and correct facts or infomlation shall be promptly submitted to the
Director by the Permittee.
4. In the event that the permitted facility fails to perform satisfactorily, the Permittee shall take
such immediate action as maybe required by the Director.
SECTION B
1. The Permittee shall collect samples from the riser pipe of CIW7 (fluid to be injected•into last
injection well in line) and from monitoring well MWTF-ICR (groundwater from the Croatan
formation) every January, April, July, and October, for the following parameters:
Total Coliform Organisms
The Permittee shall collect samples from the riser pipe of CIW7 (fluid to be injected into last

formation) every April and October for the following parameters:
Total Dissolved Solids Cadmium
Total Organic Carbon Chromium
Chloride Manganese
Fluoride Sulfate
pH Total Phosphorous
The results of the sampling and analysis shall be submitted on, Form GW-59 (Compliance
Monitoring Report Form) to the Underground Injection Control Program, Groundwater Section,
NC DENR-Division of Water .Quality, 1636 Mail Service Center, Raleigh, NC 27699-1636 and
to the Groundwater Section, Washington Regional Office, 943 Washington Square Mall,
Washington, NC 27889 every February, May, August, and November.
2. Provisions shall be made for collecting samples of the injection well system supply water, both
prior to its entrance to treatment devices and subsequent to leaving the treatment devices but
beforeentering the injection well.
3. The Permittee shall report any .monitoring or other information which indicates thatany
contaminant may cause an endangerment to an underground source of drinking water and any
noncompliance with a permit condition or malfunction of the injection system which may cause
fluid migration outside the injection zone or area of review. The information shall be provided
to the Regional Office orally within 24 hours of the occurrence and as a written submission
within five days of the occurrence. The written submission shall contain a description of the
noncompliance and its cause, the period of noncompliance, including exact dates and times, and
if the noncompliance has not been corrected, the anticipated time it is expected to continue and
any steps taken or planned to reduce, eliminate, and prevent recurrence of the noncompliance.
4. The Permittee shall retain copies or records of all monitoring information, including all
calibration and maintenance records, all original strip chart recordings for continuous monitoring
instrumentation and copies of all reports required by this permit; for a period of at least 3 years
from the date of the sample; measurement, report, or application. Records of monitoring

information shall include but not be limited to:
(A)
(B)
(C)

the date, exact place, and, time of sampling or measurements;
the individual(s) who performed the sampling or measurements;
the date(s) analyses were performed; • •
the individual(s) who perfomied the analyses;
the analytical techniques or methods used; and
the results of any such sampling, measurements, and analyses
PART VI - PERMIT RENEWAL
The Permittee shall, at least three (3) 'months prior to the expiration of this permit,
request an extension.
PART VII - CHANGE OF WELL STATUS
1. The Permittee shall provide written notification within 15 days of any change of status of an
injection well. Such a change would include the discontinued use of a well fill' injection. If a
well is taken completely out of service temporarily, the Permittee must install a sanitary seal.
If a well is not to be used for any purpose that well must be permanently abandoned "
according to 15A NCAC 2C .0113, Well Construction Standards:
2. When operations have ceased at the facility and a well will no longer be used for any
purpose, the Pennittee shall abandon that.injection well in accordance with the procedures
specified in 15A NCAC 2C .0214, including but riot -limited-to.the=following:
(A) All casing and materials may be removed prior to initiation of abandonment
procedures if the Director finds such removal will not -be responsible for, or contribute
to, the contamination of an undergroundsource of drinking water.
(B) The entire depth of each well shall be sounded before it is sealed to insure freedom
from obstructions that may interfere with sealing operations.
(C) The well shall be thoroughly disinfected, prior to sealing, if the Director determines
that failure to do so could lead to the contamination of an underground source of
drinking water.
(D)Drilled wells shall be completely filled with -cement grout, which shall be introduced
into the well through a pipe which extends to the bottom of the well and is raised as
the well is filled.
(E) In the case of gravel -packed wells in which the casing and screens have not been
removed, neat -cement shall be injected into the well completely filling it from the
bottom of the casing to the top.
(F) In those cases when, as a result of the injection operations, a subsurface cavity has

been created, each well shall be abandoned in such a manner that will prevent the
. movement of fluids into or between underground sources of drinking water and in
accordance with the terms and conditions of the permit. [eon] -
(G)The Pennittee shall submit a Well Abandonment Record (Form GW-30) as specified
in15A NCAC2C .0213(h)(1) within 30 days of completion of abandonment.
3. The written documentation required in Part VII (1) and (2) (G) shall be submitted to:

PART VIII - SPECIAL CONDITIONS
1. The quality of the injected water will not exceed the given concentrations for the following
parameters:

Total Organic Carbon: 10.0mg/L
Chloride: 55.0mg/L
Fluoride: 1.0mg/L '
Total Phosphorous: 0.15mg/L
Total Conform Organisms: 1/100m1

2. The pH of the injected water will be between 6.5 and 8:5 standard umts.
3. The temperature of the injected fluid will not exceed 35.5° C (96° F).,

media anthracite/sand filter and an ultraviolet light treatment tube system.
