On October 19, 2011, the Water Quality Regional Operations Section (Former Aquifer Protection
Section) issued a 5I groundwater remediation permit to the above for the remediation of contaminated
groundwater located at Site ST008, Pope Army Airfield, Fort Bragg, NC 28310. Expiration date for the
permit was September 30, 2014.
A review of the file indicates the Final Project Evaluation (FPE) received April 26, 2013, indicated an
exceedance of sulfate in monitoring well MW-HF1 (848 mg/L). The MCL for sulfate per 15A NCAC 2L
.0200 is 250 mg/L. This exceedance in sulfate is a result of the injection of BOS as the concentration
level increased after the injection, and was trending higher when the well was last sampled on October 31,
2012. Before closing out this permit and issuing a Completion Letter, we will need to receive a
groundwater sample collected from this well. Injectants and related reaction byproducts must attenuate to
pre -injection concentrations or groundwater standards specified in rule 15A NCAC 02C .0202, whichever
is greater, in order for this permit to be closed. Otherwise, corrective action as described Part VIII shall
be required.

I'm following up on two requests forwarded to me from you in the past several weeks regarding the owner of
injection permits associated with Pope Army Airfield sites SS019 and ST008. Apologize for the delay in
responding....
I am attaching copies of the Permit Transmittal Letters I have on file for each site issued by David Goodrich.
Each shows the owner as Gregory Bean. I don't have a phone number or email for him, but the I have provided
contract information for the people that work under/with Mr. Bean at Ft. Bragg DPW and were familiar with the
project (Christy Lowery, Steven Harris, and their supervisor Audrey Oxendine). Hope this information helps.

Dear Mr. Bean:
In accordance with the application received on August 11, 2011, and the supporting data received August 26, 2011,
August 29, 2011, September 30, 2011, and October 12, 2011, we are forwarding permit number WI0600080. This permit
is to inject BOS 200 and Trap & Treat Bacteria Concentrate to remediate groundwater contaminated with Number 2 Fuel
Oil at the facility referenced above. This permit shall be effective from the date of issuance until September 30, 2014, and
shall be subject to the conditions and limitations stated therein, including the requirement to submit a fmal project
evaluation as stated in PART VII — MONITORING AND REPORTING REQUIREMENTS. Please read the entire
permit to ensure that you are aware of all compliance requirements of the permit.
You will need to notify this office by telephone 48 hours prior to initiation of operation of the facility. In order to
continue uninterrupted legal use of the injection facility for the stated purpose, you must submit an application to renew
the permit 120 days prior to its expiration date. Please contact me at (919) 715-6162 or at david.goodrich@ncdenr.gov if

The Non -Discharge Permitting Unit is reviewing the subject permit application packages dated August 2,
2011. Additional information is required before we may continue our reviews. Please address the
following items no later than September 15, 2011:
1) In the application submitted for Site SS019 and in the application submitted for Site ST008, the site
map submitted as Attachment H (2) does not appear to have existing or proposed injection wells or
well field(s); the locations of any existing sources of potential or known groundwater contamination,
including waste storage, treatment or disposal systems within'/a mile of the injection well or well
system; all surface water bodies within ''A mile of the injection well or well system; and all existing or
abandoned wells within''% mile of the injection well(s) which penetrate the proposed injection zone,
including, but not limited to, monitoring wells and wells proposed for use as injection wells.
2) In the application submitted for Site SS019 and in the application submitted for Site ST008, the
potentiometric surface map submitted as Attachment H (3) does not appear to have existing or
proposed injection wells.
3) In the application submitted for Site SS019 and in the application submitted for Site ST008, the
contaminant plume map submitted as Attachment H (4) does not appear to have existing or proposed
injection wells.
4) In the application submitted for Site SS019 and in the application submitted for Site ST008, the cross-
section(s) to the known or projected depth of contamination submitted as Attachment H (5) do not
appear to include the horizontal and vertical extent of the contaminant plume, including
isoconcentration lines, or existing and proposed injection wells.
Please submit modified maps which include this information.
