Box 357 Graham, NC 27253 Subject: Pretreatment Review of Industrial User Pretreatment Permit - Additional Information Required Program: City of Graham WWTP: City of Graham WWTP - NPDES #: NC0021211 Alamance County Dear Ms. Smith: The Municipal Unit of the Division of Water Resources has reviewed the Industrial User Pretreatment Permit (IUP) submitted by the City of Monroe for the following Significant Industrial Users (Sills). The IUPs were received by the Division on May 31, 2021. SIU # SIU NAME 0030 Stericycle, Inc. 2001 Metal Impact East 1001 NovaFlex Hose Inc. 0007 Cintas Corporation The review indicates that more information and/or corrections are needed to determine if the IUP meets the minimum requirements of 15A NCAC 2H .0905, .0916, and .0917 and 40 CFR 403.8(f)(1)(iii). In order for the Division to complete its review of the IUP, you are required to submit the following information for each SIU by July 30, 2021: All IUPs (except Cintas): • Please have the SIU fully complete the Wastewater Pollutant Checklist in the IU application with two columns checked for each pollutant. Metal Impact East: • Add flocculation to treatment units in IUP. Therefore, as allowed by 15A NCAC 2H .0917 (g), the Division's thirty -day comment period will be suspended until the additional information is received, and at that time the full 30-day comment period will recommence. You are encouraged to notify the affected industries of the extension of the Division's comment period. Regardless of this approval action today, within 180 days of the effective date of any reissued NPDES permit (or any subsequent NPDES permit modification) the Permittee would be required to submit to the Division a written technical evaluation of the need to revise local limits (i.e., an updated IWS, or documentation of why one is not needed). This action may include revising, updating or adding to the list of Significant Industrial Users (SIU's). It is the POTW's responsibility to ensure that these objectives are consistently met.
