Quarterstone Farm Guilford County Dear Mr. Shepard: A letter of request for an Authorization to Construct was received April 16, 1987, by the Division and final plans and specifications for the subject project have been reviewed and found to be satisfactory. Authorization is hereby granted for the construction of a 0.160 MGD wastewater treatment facility consisting of approximately 200 linear feet of 8-inch CIP influent sewer on concrete piers; a flow splitter box; ODOPHOS feed equipment; dual 80,000 GPD Smith and Loveless contact stabilization treatment units, each with an influent bar screen, comminutor, 25,970 gallon reaeration tank, 19,500 gallon clarifier, 2,444 gallon sludge holding tank, and a 1431 gallon chlorine contact tank with gas chlorinator; three 300 CFM blowers to serve the entire plant; recording effluent flow meter with totalizer; and all related valves, pipes and appurtenances to serve Quarterstone Farm located in Guilford County. This Authorization to Construct is issued in accordance with Part III paragraph C of the NPDES Permit No. NC0066966 issued October 1, 1986, and shall be subject to revocation unless the wastewater treatment facilities are constructed in accordance with the conditions and limitations specified in Permit No. NC0066966. The Permittee must employ a certified wastewater operator in accordance with Part III paragraph D of the referenced permit. The sludge generated from these treatment facilities must be disposed of in accordance with G.S. 143.215.1 and in a manner approvable by the North Carolina Division of Environmental Management. The Winston-Salem Regional Office, telephone number 919/761-2351, shall be notified in advance operation of the installed system so that an in -place inspection can be made. Such notification to the Regional Supervisor shall be made during the normal office hours from 8:00 a.m. until 5:00 p.m. on Monday through Friday, excluding State Holidays. The wastewater flowrate discharged to and treated by these facilities shall not exceed 30,000 GPD until such time as the existing treatment facilities are modified to provide flow equalization. This permit shall become voidable unless the Tri-Party Agreement covering the operation and maintenance of this system between Lionmark Properties and Quarterstone Farm Association and the North Carolina Division of Environmental Management is in full force and effect.
