Division of Water Resources' Central and Regional staff has reviewed the application package
received May 18, 2021. However, additional information is required before the review may be completed.
Please address the items on the attached pages no later than the close of business on July 22, 2021.
Please be aware that you are responsible for meeting all requirements set forth in North Carolina
rules and regulations. Any oversights that occurred in the review of the subject application package are
still the Applicant's responsibility. In addition, any omissions made in responding to the outstanding items
in Sections A through F, or failure to provide the additional information on or before the above requested
date may result in your application being returned as incomplete.
Please reference the subject application number when providing the requested information. All
revised and/or additional documentation shall be signed, sealed and dated (where needed), with two paper
copies and one electronic copy submitted to my attention at the address below.

D. Operation and Maintenance Plan (O&M Plan):
1. In accordance with section E of the RLAP 06-16 application instruction form, an O&M Plan shall
be provided for all new and renewal application packages for review. Please provide the O&M Plan
which meets the 15A NCAC 02T . 1110 requirements.
E. Residuals Source Certification (FORM: RSC 06-16) Application:
1. The following municipal residuals sources did not provide a Hazardous Waste Determination as
listed in Section II.3. of the RSC 06-16 form:
a. The Town of Oak Island's Fish Factory Road Water Reclamation and Treatment Facility
(WQ0005790) in Brunswick County.
b. The Onslow Water and Sewer Authority's (OWASA) Swansboro WWTP (WQ0023261)
in Onslow County; and
c. OWASA's Summerhouse Wastewater Reclamation WWTP (WQ0029945).
Hazardous waste determinations are only excluded if the residual sources facility is less than 0.5
MGD in design flow that treat 100% non -municipal, domestic wastewater only. As these are
municipal facilities, this determination will be required as a part of this renewal and section II.3. of
the RSC 06-16 form shall be completed for each of these facilities.
