Division of Water Resources' Central and Regional staff has reviewed the application package
received April 1, 2021. However, additional information is required before the review may be completed.
Please address the items on the attached pages no later than the close of business on July 19, 2021.
Please be aware that you are responsible for meeting all requirements set forth in North Carolina
rules and regulations. Any oversights that occurred in the review of the subject application package are
still the Applicant's responsibility. In addition, any omissions made in responding to the outstanding items
in Sections A through I, or failure to provide the additional information on or before the above requested
date may result in your application being returned as incomplete.
Please reference the subject application number when providing the requested information. All
revised and/or additional documentation shall be signed, sealed and dated (where needed), with two paper
copies and one electronic copy submitted to my attention at the address below.

A. Distribution of Class A Residuals (FORM: DCAR 06-16):
1. Application Item. I1I.4. states that only 22 days of storage are provided at the facility, but the note
states that the Division requires 30 days of storage in units separate from the treatment system. If
storage below this 30-day requirement is to be suggested, the Permittee shall request to use
alternative Design Criteria and Application Submittal requirements in their response to this letter
in accordance with 15A NCAC 02T .0105(n) that demonstrates that the alternative will provide:
(1.) Equal or better treatment of the waste;
(2.) Equal or better protection of the waters of the state; and
(3.) No increased potential for nuisance conditions from noise, odor, or vermin.
Please provide a request meeting the above criteria, or provide a design that meets the 30 day
storage requirement.

E. Detailed Plans and Specifications:
1. Page 1 in Section 230902 indicates that the high -high tank level for Transfer Valve FLF96DT-V-
01 for the new storage structure is 27', and FLF31301_U502986NEW indicates that the total
height of the structure is 28'. Please note that the Division requires that all open -atmosphere
treatment lagoons, ponds and storage units shall have at least two feet of freeboard. Please update
the plans, specifications, and any calculations in Section F to reflect this freeboard requirement.
F. Engineering Calculations:
1. In accordance with 15A NCAC 02T .1104(a)(2)(C), all new and expanding residuals treatment and
storage facilities shall provide engineering calculations which include unit sizing criteria, total
dynamic head and system curve analysis for each pump, and buoyancy calculations. Please provide
signed and sealed calculations supporting the sizing of the proposed storage tank and pumps.

H. Operation and Maintenance Plan (O&M Plan):
1. The submitted O&M Plan does not immediately provide the requirements for the plan as listed in
15A NCAC 02T .1110(a). Please provide a full O&M Plan that meets these requirements for
review.
I. Residuals Source Certification (FORM: RSC 06-16):
1. The Summary for FORM: RSC 06-16 lists that the current and the maximum dry tons per year from
the residuals sources is staying the same at 41,000 dry tons per year (DT/yr). Please provide an
approximate contribution of the new reverse osmosis waste stream generated from Permit No.
NC0089621 similar to the differentiated SBM and WAS sources as listed in the Attachment A of
the current permit.
