Before we change the Owner Affiliation from Robert Massengill to Richard Kelly I want to bring to your attention that
this will make Mr. Kelly the legally responsible person for other program permits such as Non -Discharge, Wastewater,
and Sewer Collection Systems. This change will not just be for NCG080932.
If this is correct, please complete the Owner Designation/Affiliation Form.
