Staff of the North Carolina Division of Water Resources Winston-Salem Regional Office (Division) has
been trying to contact you to perform a routine compliance inspection of the wastewater irrigation system
serving the residence located at 627 Willie Pace Rd. Unfortunately, the phone numbers that we have on
file are no longer in service. On May 27, 2021, Division staff made a site visit in an attempt to conduct
the routine inspection but there was no answer at the door of the residence. Division staff left a door
hanger on the front door of the residence requesting that you contact the Winston-Salem Regional Office
to schedule an inspection. To date, our office has not received any response.
As a reminder, Permit No. WQ0029555, Condition V. 3. states that Division representatives may inspect
the wastewater treatment and irrigation facilities for determining compliance with the permit. Our office
is mandated to conduct routine inspections of such facilities with minimum frequency requirements. The
most recent inspection of the subject facility was conducted on March 4, 2015. Please contact Patrick

Inspection Summary:
On May 27, 2021, WSRO staff conducted a site visit to the subject facility in an effort to conduct a routine compliance
inspection. The phone numbers on file are all disconnected or no longer in service. There was no answer at the door at the
time of the site visit. There was one car present in the driveway, and it appeared the area was being maintained (i.e., lawn
was mowed, spray field was mowed, etc.). A door hanger was left on the front door requesting the owner or resident to
contact the WSRO to schedule an inspection.
