In accordance with your application received March 1, 2013, we are forwarding herewith Permit No.
WQ0036402, dated March 12, 2013, to the City of Mount Holly for the construction and operation of the
subject wastewater collection system extension. This permit shall be effective from the date of issuance
until rescinded, and shall be subject to the conditions and limitations as specified therein. This cover
letter and supplement shall be considered a part of this permit and are therefore incorporated therein
by reference.
Please pay particular attention to the following conditions contained within this per
Condition 11.4:
Requires that the wastewater collection facilities be properly operated and maintained
in accordance with 15A NCAC 2T .0403 or any individual systemwide collection system
permit issued to the Permittee.
It shall be responsibility of the City of Mount Holly to ensure that the as -constructed project meets the
appropriate design criteria and rules. Failure to comply may result in penalties in accordance with North
Carolina General Statute §143-215.6A through §143-215.6C, construction of additional or replacement
wastewater collection facilities, and/or referral of the North Carolina -licensed Professional Engineer to
the licensing board.
If any parts, requirements, or Iimitations contained in this permit are unacceptable, you have the right
to request an adjudicatory hearing upon written request within 30 days following receipt of this permit.
This request must be in the form of a written petition, conforming to Chapter 150B of North Carolina
General Statutes, and filed with the Office of Administrative Hearings, 6714 Mail service Center, Raleigh,
NC 27699-6714. Unless such demands are made, this permit shall be final and binding.

In accordance with the provisions of Article 21 of Chapter 143, General Statutes of North Carolina as
amended, and other applicable Laws, Rules, and Regulations, permission is hereby granted to the
City of Mount Holly
Gaston County
for the construction and operation of approximately 981 linear feet of 3-inch force main to serve the
MICS Force Main, and the discharge of 0 gallons per day of collected domestic wastewater into the City
of Mount. Holly's existing sewerage system, pursuant to the application received March 1, 2013, and in
conformity with 15A NCAC 2T; the Division's Gravity Sewer Minimum Design Criteria adopted February
12, 1996 as applicable; the Division's Minimum Design Criteria for the Fast -Track Permitting of Pump
Stations and Force Mains adopted June 1, 2000 as applicable; and other supporting data subsequently
filed and approved by the Department of Environment and Natural Resources and considered a part of
this permit.
This permit shall be effective from the date of issuance until rescinded and shall be subject to the
specified conditions and limitations contained therein.

mentioned herein. Division approval is based on acceptance of the certification provided by a North

Department are advised that, no certificates of occupancy are to be granted prior to the submnitta} of sealed

11. STANDARD CONDITIONS
l. This permit shall not be transferable. In the event there is a desire for the wastewater collection facilities to
change ownership, or there is a name change of the Permittee, a formal permit request shall be submitted to
the Division accompanied by documentation from the parties involved, and other supporting materials as may
be appropriate, The approval of this request shall be considered on its merits and may or may not be
approved.
2 This permit shall become voidable unless the wastewater collection facilities are constructed in accordance
with the conditions of this permit, 15A NCA[ ZT; the Division of VVaLer, Quality's (Division) Gravity Sewer
Minimum Design Criteria adopted February12, l996asapplicable; the Division's Minimum Design Criteria for
the Fast -Track Permitting ofPump Stations and Force Mains adopted June 1, ZD00asapplicable; and other
supporting materials unless sped#ca(ymentioned herein.
3. This permit shall be effective only with respect to the nature and volume of wastes described in the
application and other supporting data,
4, The wastewater collection facilities shall be properiymaintaioed and operated atall times, The Pemmdtee
shall maintain compliance with an individual system -wide collection system permit for the operation and
maintenance of these facilities as required by 15A m[AC 2T .0403. If an individual permit is not required, the
following performance criteria shall be met as provided in lSA NCAC 2T.0483�-
a. The sewer system shall be effectively maintained and operated at all times to prevent discharge to land or
surface waters, and to prevent any contravention of groundwater standards or surface water standards,
b, A map of the sewer system shall be deveioped and shall be actively maintained,
c, An operation and maintenance p�am including pump station inspection frequency, preventative
maintenance schedule, spare pmnz inventory and overflow response has been developed and
imp|emented.
d, Pump stations that are not connected to otdemetrysystem shall be inspected every day (i.e- 365 days
per year), Pump stations that are connected to a te|emetryryaen shall be inspected at least once per
week,
e. High -priority sewer lines shall he inspected at least once per every six -months and inspections are
documented,
t A general observation of the entire sewer system shall be conducted at least once per year.
& Overflows and bypasses shall be reported tothe appropriate Division regional office in accordance with
ISA NQ\[ 28 .0506(o)' and public notice shall be provided as required by North Carolina General Statute

distribution of educational materials for both commercial and residential users and the legal means to
require grease interceptors atexisting establishments, The plan shall also include legal means for
inspections of the grease interceptors, enforcement for violators and the |ega| means to control
grease entering the system from other public and private satellite sewer systemc

distribution of grease education materials to users of the collection system by the permitrpe or its

If the permit is issued to a private aruby with an Operational Agreement, then u copy ofthe Articles of
Incorporation, Declaration s/Covenanm/Rest rbtions,and Bylaws that have been appropriately filed with the
applicable County's Register of Deeds office shall be submitted with the certification.

compliance with this permit (including pipe length increases afI0% orgreater, increased flow, pump station
design capacity design increases of 5% or greater, and increases in the number/type o/ connections),
regulations, or ,he Minimum Design Criteria, Requested modifications orvariances to the Minimum Design
Criteria will be reviewed on a case -by -case basis and each on its own mark. Please note that variances to the
Minimum Design Criteria should be requested and approved during the permitting process prior to
construcdon. After -construction requests are discouraged by the Division and may not be approved, thus
requiring replacement ur repair prior to certification & activation,

9, Failure to abide by the conditions and limitations contained in this permit, 1SA NCAC 2T; the Division's Gravity

Fast -Track Permitting of Pump Station and Force Mains adopted June I, 2000 as applicable; and other
supporting materials may subject the Porm|tteeto an enforcement action by the Division, in accordance with
North Carolina General Statutes §I43-215.15A through 4143'2l5.6C,construction ofadditional u/replacement
wastewater collection facilities, and/or referral of the North Carolina -licensed Professional Engineer tuthe
licensing board.
lO. In the event that the wastewater collection facilities fail to perform satisfactorily, including the creation of
nuisance conditions, the PermiUeeshall take immediate corrective action, including those asmay berequired
by this Ulvismn, such as the construction of additional or replacement facilities.
11. The issuance of this permit shall not exempt the Pemnuee from complying with any and all statutes, rules,
regulations, or ordinances that may be imposed by other government agencies (local, state and federal) which
have jurisdiction, including but not limited to applicable river buffer rules in I5A N[A[ 28 .0200, erosion and
sedimentation control requirements in I5A N[A[Ch. 4 and under the Division's General Permit NC6010000'
and any requirements pertaining towetlands under I5AN[AC 28 .0200and I5A N[4[2M .9500�

The City of Mount Holly had an Annexation study completed in January
of 2009 to determine the potential of annexing the .former Duke Village
also known as .Annexation area 8. The proposed 3" sanitary sewer force
main would serve the proposed school and this annexation area.
The proposed drainage basin consists of the following uses:
(All flows are based on I5A NCAC 02T.0114)
Proposed to be .currently served by the force main.
-Mountain Island Charter School with 1,200 students
(15 gallons/student with gym and showers)
1,200 students x 15 gpd/student = 18,000 gpd
Future Annexation and Connections
-Proposed I\it Holly Fire Station assuming 6 on -site employees per shift
(50 gallons/person/shift)
6 employees x 3 shifts x 50 gallons/person/shift = 900 gpd
-49 Existing Residential Houses that average 3 bedrooms per house
(120 gallons per day per bedroom)
49 houses x 3 bedrooms x 120 gpd/bedroom 17,640 gpd
-Existing Food Service Building (Approximately 3,000 sf)
(Specialty food stand is 50 ga1/100 sf of floor space)
.3000 sf x 50 gallons / 100 sf = 1500 gpd
Existing vacant land is 25.67 acres
(Assuming future development of 1 house per acre and 3 bedrooms)
25 houses x 3 'bedrooms x 1.20 gpd/bedroom = 9,000 gpd
Ultimate Proposed Sanitary Sewer Flow for the drainage basin is
47,040 gpd.
Based on an average daily flow of 47,040 gpd and a 2.5 peaking factor the
future anticipated flow is 81.67 gpm. Assuming a pump station that
would deliver a peak pumping rate of 82 gpm the anticipated velocity in a
3" HDPE force main 'would be 3.73 fps (see next page for calculations).
This velocity would bring the sanitary sewer force main into compliance
with the Minimum Design Criteria for the Permitting of Pump Stations
and Force Mains.
