On April 2, 2013 the Division of Water Quality (DWQ) received information for the above referenced
project. The DWQ has determined that the application was incomplete and/or provided inaccurate
information as discussed below.
Additional Information Requested:
® A review of the application reflects the need for a check in the amount of $240. Check # 1385
for $570 is enclosed. DWQ has been in touch with Ms. Jessica Mallindine of the
Environmental Resources section and she will be forwarding to DWQ four (4) signed copies of
the Preconstruction Notification form (PCN) along with the appropriate $240 fee.
Please submit this information within 30 calendar days of the date of this letter. This letter only addresses
the application review and does not authorize any impacts to wetlands, waters or protected buffers.
Please be aware that any impacts requested within your application are not authorized (at this time) by the
